肺结核合并糖尿病与单纯性肺结核临床疗效对比

Clinical effects of pulmonary tuberculosis complicated with diabetes and simple pulmonary tuberculosis

目的探讨在肺结核合并糖尿病时控制糖尿病对肺结核治疗的疗效及影响。方法对滨州市中心医院（结防院）收治的40例肺结核合并糖尿病患者和38例单纯性肺结核患者的临床资料进行回顾性分析，比较两组痰菌阳性率、排菌量、空洞情况等。结果两组治疗2个月和6个月后痰菌转阴率比较差异均未见统计学意义（P〉0．05）。血糖控制良好的观察组患者在空洞闭合、肺部病灶吸收等指标上与对照组比较差异未见统计学意义（P〉0．05），而血糖控制不良的观察组患者则与对照组比较差异有统计学意义（P〈0．01）。结论糖尿病因素会加重肺结核病情，并影响抗结核疗效。对肺结核合并糖尿病患者应进行早期规则抗痨治疗，积极控制血糖，可取得与单纯性肺结核治疗同样的效果。

Objective To investigate the effects of diabetes control on pulmonary tuberculosis m the treatment of pulmonary tuberculosis complicated with diabetes. Methods A total of 40 cases of pul- monary tuberculosis with diabetes and 38 patients with pulmonary tuberculosis were retrospectively ana- lyzed, the sputum positive rate, discharge of bacteria amount, empty was compared. Results After 2 months and 6 months, there was no significant difference in the sputum negative conversion rate between the two groups. Good blood glucose control in patients in the observation group in the cavity closure, ab- sorption of lung lesions were compared with the control group, there was no significant difference （ P 〉 0. 05 ） , and poor control of blood sugar of the patients in the observation group had significant difference with the control group （P 〈 0.01 ）. Conclusions Diabetes can aggravate the condition of TB, and inter- ference to the anti-tuberculosis treatment. In patients with pulmonary tuberculosis complicated with diabe- tes should be given early regular anti-tuberculosis treatment, control of the blood glucose, can made the same effect with simple pulmonary tuberculosis treatment.
