五虎汤合葶苈大枣泻肺汤加减治疗新生儿支原体肺炎痰热闭肺证40例疗效观察

Clinical curative effect of treatment with Wuhu Decoction combined with modified Tingli Dazao Xiefei Decoction treating neonatal mycoplasma pneumonia with phlegm heat obstructing lung syndrome

目的探讨五虎汤合葶苈大枣泻肺汤加减辨证治疗新生儿支原体肺炎痰热闭肺证的临床疗效及对相关细胞因子的影响。方法选择我院收治的新生儿支原体肺炎痰热闭肺证患儿80例,随机分为治疗组和对照组各40例。对照组给予常规治疗,治疗组在常规治疗基础上给予五虎汤合葶苈大枣泻肺汤加减辨证治疗,观察并记录2组患儿的退热时间、肺部啰音消失时间以及住院时间,并于治疗7 d后对临床疗效以及相关细胞因子的变化进行统计分析。结果治疗组患儿退热时间、肺部啰音消失时间及住院时间均明显短于对照组,差异均有统计意义(P<0.05);治疗组总有效率明显高于对照组,差异有统计意义(P<0.05);治疗7 d后,2组患儿血清IL-6,IL-8和TNF-α水平均较治疗前明显降低(P<0.01),且治疗组改善程度更为明显,差异均有高度统计意义(P<0.01)。结论五虎汤合葶苈大枣泻肺汤辨证治疗新生儿支原体肺炎痰热闭肺证不仅有助于改善临床症状,提高临床疗效,而且对于提高患儿的免疫功能也具有重要作用。

Objective To investigate the clinical curative effect and influence on related cytokines of syn- drome deferential treatment with Wuhu Decoction combined with modified Tingli Dazao Xiefei Decoction treating neonatal mycoplasma pneumonia（ phlegm-heat obstructing lung syndrome）. Methods 40 cases of patients in our hospital with neonatal mycoplasma pneumonia with phlegm heat obstructing lung syndrome were randomly divided into a treatment group and a control group. The control group were given conventional treatment while the treatment group were given deferential treatment with Wuhu Decoction combined with modified Tingli Dazao Xiefei Decoction on the basis of conventional treatment. Observed and recorded the antifebrile time,lung rale disappear time and hos- pital stay time of the two groups and evaluated the clinical curative effect and the change of related cytokines after treatment for 7 days. Results The antifebrile time, lung rale disappear time and hospital stay time of the treatment group were significantly shorter than those in the control group, and the difference was statistically significant （P〈 0.05 ）. The overall response rate of the treatment group was significantly higher than that of the control group, and the difference had statistical significance （P〈0.05）. After treatment for 7 days, the serum IL-6, IL-8 and TNF-cL level of the children in the two groups were significantly lower than those before treatment（P〈0.01 ） ,and the treat- ment group improved better than the control group, the difference had statistical significance （P〈O. O1 ）. Conclu- sion When treating neonatal mycoplasma pneumonia with phlegm heat obstructing lung syndrome, deferential treatment with Wuhu Decoction combined with modified Tingli Dazao Xiefei Decoction not only can improve the clinical symptoms and clinical curative effect, but also can improve children＇ s immune function.
