卡前列素氨丁三醇注射液与缩宫素在前置胎盘产后出血的临床预防效果比较

Comparison of Clinical Preventive Effect on Postpartum Hemorrhage in Placenta Previa between Carboprost Tromethamine Injection and Oxytocin Injection

目的比较卡前列素氨丁三醇注射液与缩宫素对预防前置胎盘产后出血的临床效果。方法选取2009年10月至2013年12月洪湖市妇幼保健院治疗的87例胎盘前置的孕产妇,按随机数字表法分为缩宫素组(44例)和卡前列素氨丁三醇注射液组(43例)。缩宫素组应用缩宫素治疗,卡前列素氨丁三醇注射液组应用卡前列素氨丁三醇注射液治疗(250μg,子宫体注射),比较两组孕产妇在术中、术后2、24 h的出血情况、血红蛋白变化及不良反应发生情况。结果缩宫素组分娩术中、术后2 h、术后24 h出血量分别为(455±36)m L、(423±46)m L、(357±24)m L,卡前列素氨丁三醇注射液组分娩术中、术后2 h、术后24 h出血量分别为(403±31)m L、(227±43)m L、(193±23)m L。与缩宫素组相比,卡前列素氨丁三醇注射液组各阶段出血量均较少,差异有统计学意义(P<0.05)。两组均出现血压一过性升高、恶心、面部潮红等不良反应,但两组不良反应发生率比较差异无统计学意义(P>0.05)。结论前置胎盘孕产妇注射卡前列素氨丁三醇注射液对预防产后出血具有良好的作用。

Objective To compare the prevention and treatment effect between carboprost tromethamine injection and oxytocin injection on postpartum hemorrhage in placenta previa .Methods A total of 87 cases of pregnant women with placenta previa admitted in Honghu Maternal and Child Health Care Hospital from Oct.2009 to Dec.2013 were selected and divided into two groups according to random number table.Patients in oxytocin group（44 cases） were treated with oxytocin,while patients in carboprost tromethamine group（43 cases） were treated with carboprost tromethamine injection（250 μg,uterine body injection）.Blood loss vol-umes and hemoglobin level changes during the operation,2 hours and 24 hours after operation of the two groups were compared.Results The blood loss volumes during the operation,2 hours and 24 hours after operation of the oxytocin group were （455 ±36） mL,（423 ±46） mL and （357 ±24） mL respectively,while those of the carboprost tromethamine group were （ 403 ±31 ） mL, （ 227 ±43 ） mL and （ 193 ±23 ） mL respectively.The blood loss volumes of the carboprost tromethamine group were significantly less than the oxytocin group at different phases mentioned above （ P〈0 .05 ） .There were incidences of adverse reactions such as transient rise in blood pressure,nausea and facial blushing in two groups,although there were no sta-tistically significant differences of such incidences between the two groups（P〉0.05）.Conclusion Prophy-lactic use of carboprost tromethamine injection in pregnant women with placenta previa can significantly reduce the risk of postoperative bleeding.
