促进胃肠道手术后胃肠功能恢复临床观察

Recovery of gastrointestinal function after surgery

目的观察运用不同方法治疗胃肠道术后排气排便不能或腹胀患者的临床效果。方法 2012年8月至2014年8月胃肠手术后腹胀或者4 d未排气排便的186例患者,根据治疗方法不同将其分为两组。开塞露组90例患者运用开塞露纳肛,杜密克组96例患者运用口服杜密克治疗。采用SPSS 18.0统计学软件进行数据分析,排气排便资料以(x珋±s)表示,进行t检验;疗效评价采用χ2检验,P<0.05为差异有统计学意义。结果在促进排气方面,杜密克组(47.65±15.18)h优于开塞露组(52.73±15.23)h,P=0.024;在促进肛门排便方面,杜密克组(48.46±13.38)h早于开塞露组(53.69±14.07)h,P=0.01;杜密克组总有效率(97.92%)高于开塞露组(90.00%)(χ2=5.232,P=0.022),以上两组差异均有统计学意义。结论口服杜密克治疗术后胃肠功能恢复不全的临床效果较为显著,值得在临床上推广使用。

Objective To investigate the clinical outcome of different methods in treating patients with delayed exhaust defecation and bloating after gastrointestinal surgery. Methods From August 2012 to August 2014, 186 patients with bloating or 4 days after gastrointestinal surgery did not exhaust defecation. They according to the different treatment methods were divided into two groups： 90 patients in a glycerol enema group and 96 patients in a lactulose group. The data were analyzed using SPSS 18.0 statistical software, exhaust defecation data were expressed as mean ± standard deviation and analyzed by Students t test. A P value 〈 0. 05 was considered statistically significant. Results For exhaustion, the time in the lactulose group was （47. 65 ± 15. 18） hours, whereas that of the glycerol enema group was （52. 73 ± 15.23） hours （P = 0. 024）; the time for defecation in the lactulose group was （48.46 ± 13.38） hours, whereas that in the glycerol enema group was （53.69 ± 14.07） hours （P = 0. 01 ）. In the lactulose group, the total effective rate was 97.92%, whereas in the glycerol enema group, it was 90.00% ; the total effective rate of the lactulose group was higher than that of the glycerol enema group （X2 = 5. 232, P = 0. 022 ）. There were significant differences in the two groups, Conclusion Oral lactulose treatment for delayed postoperative recovery of gastrointestinal function is more significant and is worth of wider clinical application.
