两种红细胞制品治疗自身免疫性溶血性贫血的效果比较

Comparison on the efficacy of two kinds of transfusion of red blood cell products on autoimmune hemolytic anemia

目的观察2种红细胞制品治疗自身免疫性溶血性贫血的疗效,为临床输血治疗提供依据。方法选择2012年2月-2014年2月在郑州某3家医院收治的自身免疫性溶血性贫血患者60名,随机分为A、B组,A组输注去白悬浮红细胞,B组输注洗涤红细胞,观察2组患者输血疗效及输血不良反应。结果 A组30名患者,有效16例,部分有效8例,无效6例,总有效率为80.0%,B组30名患者,有效15例,部分有效8,无效7例,总有效率为76.7%,2组比较差异无统计学意义(P>0.05),输血不良反应相似,发生率均较低,2组均未出现明确的溶血性输血反应。结论去白悬浮红细胞治疗自身免疫性溶血性贫血疗效确切,而且不会增加输血不良反应,值得临床考虑。

Objective To compare the efficacy of two kinds of red blood cell products transfusion in autoimmune hemolytic anemia （AIHA） and to provide evidence for clinical blood transfusion. Methods Sixty patients with AIHA were selected from three hospitals in Zhengzhou from February 2012 to February 2014. They were randomly divided into two groups A and B. Leukocyte RBCs suspension were transfused to patients in A group, while washed RBCs were transfused to patients in B group. Transfusion efficacy and adverse transfusion reactions were observed in two groups. Results Among thirty patients in A group, their transfusion efficiency were 16, with a fractional efficiency of 8, reaching a total efficiency rate of 80%. Among the thirty patients in B group, their transfusion efficiency were 15, with a fractional efficiency of 8, reaching a total efficiency rate of 76.7%. There was no significant difference on transfusion efficacy between the two groups （P 〉 0. 05）. The incidence of adverse transfusion reactions in two groups were similar. There was no clear hemolytic transfusion reaction in two groups. ConclUsion Transfusion of leukocyte RBCs suspension can be a therapy for the patients with AIHA without the repercussion of adverse transfusion reactions. These types of RBC product were worthy for clinical application.
