广西县级妇幼保健院医疗工作质量综合评价分析

Applying the TOPSIS Method to Evaluate the County-level Maternal and Child Health Institutes' Comprehensive Medical Work Quality in Guangxi

目的应用TOPSIS法对2008—2010年广西县级妇幼保健院的医疗工作质量进行综合评价分析。方法通过自编调查表收集广西67家县级妇幼保健院2008—2010年门诊人次、出院人次、治愈好转率、病床使用率、平均住院日、入出院诊断符合率和手术前后诊断符合率等数据,应用TOPSIS法对广西县级妇幼保健院的工作质量进行综合评价。结果 2008—2010年广西县级妇幼保健院的门诊人次分别为53 132、60 613和66 156人次;出院人次分别为3 082、3 448和3 950人次;治愈好转率分别为96.79%、97.63%和96.77%;病床使用率分别为74.85%、77.51%和78.60%;平均住院日分别为4.49、4.52和4.62天;入出院诊断符合率分别为94.76%、95.14%和95.06%;手术前后诊断符合率分别为95.49%、95.65%和95.62%。Ci值分别为0.134 9、0.523 7和0.877 0。结论除治愈好转率出现波动外,其他反映医疗工作质量的各个指标数均逐年增长;医疗工作质量呈现逐年上升的趋势,2010年的医疗工作质量最好,2009年次之,2008年最差。评价结果与实际情况相符,TOPSIS法在科学综合评价医疗工作质量中具有很好的应用价值。

OBJECTIVE Applying the TOPSIS method to evaluate the county maternity and child health institutes＇ comprehensive medical work quality in Guangxi from 2008 to 2010. METHODS The outpatient number, discharge number, cure rate, sickbed utilization rate, average length of stay, the admission and discharge diagnosis coincidence rate and diagnosis coincidence rate before and after operation were collected from 67 county maternity and child health institute in Guangxi from 2008 to 2010 by the self-made ques- tionnaires. TOPSIS method was used to evaluate the county maternity and child care medical comprehensive work quality. RESULTS The outpatient numbers of Guangxi county-level maternity and child health institutes from 2008 to 2010 were 53 132, 60 613 and 66 156 respectively, discharge numbers were 3 082, 3 448 and 3 950 respectively; cure rates were 96.79%, 97.63% and 97.77% respectively; sickbed utilization rateswere 74.85%, 77.51% and 78.60% respectively; average length of stay were 4.49, 4.52 and 4.62 days respectively; the admission and discharge diagnosis coincidence rates were 94.76%, 95.14% and 95.06% and diagnosis coincidence rate before and after operation were 95.49%, 95.65% and 95.62% respectively. C~ values were 0.134 9, 0.523 7 and 0.877 0 respectively. CONCLUSION In addition to the cure ratewas fluctuated, all the other medical work quality indexs were in- creased year by year; medical work quality showed a trend of rising year by year, in 2010 the medical work of the best quality, the second in 2009, the worst in 2008. Evaluation result was consistent with the actual situation, the TOPSIS method had a very good application value in comprehensive evaluatethe medical quality scientifically.
