张玉琴从郁(瘀)毒损络分期辨治糖尿病肾病

张玉琴教授认为"久病入络"和"久痛入络",郁(瘀)毒损伤肾络为糖尿病肾病的主要病机,提出了消渴郁(瘀)毒学说,"郁"在前,"瘀"在后,"郁"范围广,"瘀"比较局限;"郁"指气郁,"瘀"指血瘀。气郁、酒郁、食郁最终导致血瘀,运用解毒通络法治疗。早期以气阴两虚、瘀血阻络为主,益气养阴,活血即是解毒;终末期则以阴阳两虚、湿毒上逆突出,形成"痰瘀""湿瘀"等,温阳降浊、化瘀利湿即是解毒。通络之法,以化瘀通络、利湿通络及虫类药搜风通络为主。
