糖皮质激素在调节类风湿关节炎患者多项血清因子中的效果观察

Observe the Effect of Glucocorticoids for the Disease-related Factors in Serum of Patients with Rheumatoid Arthritis

目的:分析糖皮质激素对类风湿关节炎患者血清炎症因子及基质金属蛋白酶含量的影响。方法:将180例类风湿关节炎患者随机分为两组,各90例。对照组采用基础治疗,观察组则在基础治疗的前提下加载糖皮质激素。比较治疗前及治疗后3、6个月,血清炎症因子(TNF-α、IL-8和IP-10)和基质金属蛋白酶(MMP-2、MMP-3、MMP-9)水平差异。结果:治疗后3、6个月,观察组患者血清TNF-α、IL-8、IP-10和基质金属蛋白酶(MMP-2、MMP-3、MMP-9)水平均显著低于对照组,P<0.05;治疗6个月后,观察组患者疗效达到ACR50、ACR70比例显著高于对照组,P<0.05。结论:糖皮质激素能有效抑制炎症因子和金属基质蛋白酶释放,从而有利于改善类风湿关节炎患者的预后。

Objective：To analyze the effect of glucocorticoids for the inflammatory cytokines and matrix metalloproteinases in serum of patients with rheumatoid arthritis（RA）.Methods：180patients with RA were randomly divided into control group and observation group,each group got 90 cases,those in control group were took treatment group and those in observation group were combined with glucocorticoids.Then the serum levels of inflammatory cytokines（TNF-α,IL-8and IP-10）and matrix metalloproteinase（MMP-2,MMP-3,MMP-9）of two groups before the treatment and at third,sixth month after the treatment were detected and compared.Results：3and 6months after treatment,the serum levels of inflammatory cytokines（TNF-α,IL-8and IP-10）and matrix metalloproteinase（MMP-2,MMP-3,MMP-9）of observation group were all lower than those of control group,all P〈0.05.6months after treatment,the efficacy reached ACR50,ACR70 proportion of observation group were all higher than those of control group,all P〈0.05.Conclusion：Glucocorticoids can inhibit the release of inflammatory cytokines and matrix metalloproteinase,thus contributing to improve the prognosis of patients with rheumatoid arthritis.
