通阳化瘀、调和营卫法对慢性阻塞性肺疾病急性加重期的疗效

Removing Extravasated Blood and Harmonizing Yingfen and Weifen for Acute Exacerbations of Chronic Obstructive Pulmonary Disease

目的探讨中西医结合治疗慢性阻塞性肺疾病急性加重期(AECOPD)的临床疗效。方法将60例AECOPD的患者按不同的治疗方法分为2组:治疗组(30例)和对照组(30例)。采用"GOLD"发布《慢性阻塞性肺疾病全球倡议》中AECOPD的规范化治疗方案,吸入支气管舒张剂,视病情使用抗生素、支气管解痉剂、化痰药、扩血管药、强心药等。治疗组在上述西医治疗基础上联合中药通阳化瘀、调和营卫方。观察2组患者临床疗效,BODE评价系统指标:FEV1、呼吸困难分级、6分钟步行距离等治疗前后的变化。结果治疗组总有效率较对照组明显改善(P<0.05)。2组治疗后BODE评分较治疗前均明显改善(P<0.05),治疗组治疗后BODE评分较对照组明显改善(P<0.05)。结论通阳化瘀、调和营卫法治疗AECOPD可明显改善患者临床疗效、BODE评分,且无明显的不良反应,是一种治疗AECOPD较理想的方法。

ABSTRACT:Objective To investigate the clinical effect of integrated Chinese and Western medi-cine on acute exacerbations of chronic obstructive pulmonary disease(AECOPD).Methods Sixty patients with AECOPD were divided into two groups:treatment group and control group,with 30 patients in each group.According to the Global Initiative for Chronic Obstructive Lung Disease (GOLD),all patients were given standardized treatment for AECOPD,including inhalation of bronchodilators,appropriate antibiotics,bronchial spasmolytic agents,expectorant drugs,vasodila-tors,and cardiotonic drugs.In addition,patients in treatment group were treated with removing extravasated blood and harmonizing yingfen and weifen.Clinical efficacies were assessed in both groups.Moreover,BODE indexes,including forced expiratory volume in 1 second(FEV1),dyspnea grade and 6-minute walk distance,were observed before and after treatment.Results BODE score was significantly improved after treatment in both groups (P<0.05 ).Compared with control group,the total effective rate and BODE score were significantly improved in treatment group (P<0.05).Conclusion Removing extravasated blood and harmonizing yingfen and weifen can&nbsp;improve clinical efficacy and BODE score without obvious adverse reactions in patients with AE-COPD.Therefore,this method is an ideal treatment for AECOPD.
