摘要
目的探讨康复训练计划表在人工全髋关节置换(THA)术后功能锻炼中的应用效果,为THA术后护理提供指导。方法选择2013年6月至2014年6月出院的人工THA术后90例患者,入院后随机分为对照组和观察组各45例,两组均进行常规健康宣教,观察组在此基础上按康复训练计划表进行锻炼。比较两组患者出院时、术后3、6个月Harris髋关节评分、Barthel指数及出院时并发症情况。结果出院时两组患者Harris髋关节评分差异无统计学意义(P>0.05),术后3、6个月时,患者Harris髋关节评分观察组高于对照组(P<0.05)。出院时两组患者Barthel指数差异无统计学意义(P>0.05),术后3、6个月时,患者Barthel指数评分均显著高于对照组,差异有统计学意义(P<0.05);出院时对照组有10例发生并发症,占22.22%,观察组有3例发生并发症,占6.66%;两组比较有统计学差异(P<0.05)。结论康复训练计划表的应用,使锻炼内容具体化,操作规范化,又能根据患者情况不同行个体化。起到术后延续指导和督促训练的作用,提高功能锻炼效果,促进关节功能康复。
Objective To discuss the effects of rehabilitation schedule on the functional training exercises after total hip arthoplasty( THA),and provide guidance for postoperative nursing of THA.Methods Ninety patients who underwent THA between June 2013 and June 2014 were selected and randomized into the control group and the observation group( 45 patients each). In addition to the regular health education for both of the two groups,the rehabilitation schedule was adopted in the observation group to guide the exercises. The comparisons of the two groups were performed in the complications,Barthel index and Harris hip scores at the time of hospital discharge,three and six months after the surgery.Results The differences in Harris hip scores and Barthel index between the two groups were not significant at the time of hospital discharge( P〉 0. 05),while the Harris score and Barthel index of the observation group were both higher than those of the control group three and six months after the surgery( P〈 0. 05).At the time of hospital discharge,complications were found in 10 cases of the control group and three cases of the observation group; the incidences were 22. 22% and 6. 66% respectively,which was significant different( P〈 0. 05). Conclusion The application of rehabilitation schedule can solidify,normalize and individualize the functional exercises according to different patients,provide guidance and supervision for the postoperative training,so as to improve the joint function.
