多层螺旋CT在胆囊癌临床诊断中的应用

Diagnostie value of multislice spiral computed tomography in gallbladder carcinoma

目的:探讨多层螺旋CT对胆囊癌的诊断和鉴别诊断,以提高胆囊癌的诊断水平。方法:回顾性分析经手术病理证实的20例胆囊癌患者资料的CT表现,并与手术病理相对照,分析胆囊癌误诊原因。结果:20例病例中肿块型9例(占45%);腔内结节型4例(占20%);胆囊壁增厚型7例(占35%),合并肝脏及其他脏器受侵、肝脏及淋巴结转移、肝内外胆管扩张、胆囊结石等间接征象。结论:CT扫描不仅能正确诊断大多数胆囊癌,而且可鉴别肿瘤类型;同时能仔细分析胆囊及周围的结构和器官的改变,有助于提高胆囊癌的CT诊断水平。

Objective： To investigate the CT findings of gallbladder carcinoma ang toelevate its diagnostic level. Methods： CT findings of 20 patiets with surgically and pathologically proved gallbladder carcinoma were retrospectively analyzed. Results： Among 20 cases, gallbladder carcinoma of soft tissue mass type was found in 9 cases（45% ）, intracystic nodule type in 4（20%）, and thickened wall type in 7 （55%）. Other CT findings included direct invasion of liver and other adjacent organs, hematatogenous liver matastases, meta-static lym-phadenopathy, both intra and extra-hepatic biliary dilataion and gallstone. Conclusion： CT seans can ontonly correctly diagnose the most patients with gallbladder carcinoma but differentiate the tumor type. It is helpful to stady carefully changes of the gallbladder and its surrounding structure and organs for the diagnosis of gallbladder carcinoma on CT.
