BNP、hs-cTnT在新生儿持续肺动脉高压心肌损伤检测中的临床意义 被引量:5

摘要 探讨血浆B型尿钠肽(BNP)、高敏肌钙蛋白T(hs-cTnT)在新生儿持续肺动脉高压(PPHN)心肌损伤早期诊断和治疗中的应用。本研究以60例PPHN患儿和40例正常新生儿为研究对象,对BNP、hs-cTnT的水平与肺动脉收缩压(PASP)的相关性进行研究。结果表明,PPHN组患儿BNP、hs-cTnT水平显著高于正常新生儿(P<0.05)。治疗2周后,患儿BNP、hs-cTnT水平均较治疗前明显降低(P<0.05)。BNP、hs-cTnT水平与PASP呈显著正相关(P<0.01)。BNP的敏感性及特异性分别为88.36%、90.41%,而hs-cTnT的敏感性及特异性分别为82.67%、85.29%。因此,BNP、hs-cTnT可作为心肌损伤早期诊断和疗效评估的重要指标。 To investigate the application of plasma brain‐type natriuretic peptide and high sensitivity of cardiac troponin T for early diagnose and treatment in myocardium injure of persistent pulmonary hypertension of the newborn (PPHN) .In this study ,60 PPHN and 40 normal neonates were chosen to investigate the correlation of BNP ,hs‐cTnT and pulmonary artery systolic pressure . The results showed that plasma levels of BNP , hs‐cTnT in case group were statistically significantly higher than those in control group (P〈 0 .05) .After being treated for two weeks ,the levels of BNP ,hs‐cTnT dropped significantly compared with that before treatment .There was a positive correlation between the plasma levels of BNP ,hs‐cTnT and PASP .The sensibility and specificity of BNP were 88 .36% and 90 .41% ,while those for hs‐cTnT were 82 .67% and 85 .29% .Therefore ,early PPHN may onset by myocardium injure ,and the BNP ,hs‐cTnT were important indexes for early diagnose and treatment in myocardium injure of persistent pulmonary hypertension of the newborn .
