摘要
目的:比较精神分裂症患者一级亲属(高危人群)和家族史阴性、首发未用药患者及正常人群之间大脑结构形态的差异。方法:应用基于体素的形态学研究方法(VBM),对满足入组标准的高危组(患者的一级亲属且无精神症状)10例、首发组(首次发病未用药且家族史阴性)10例及正常人群(正常对照组)10名进行头颅核磁共振扫描和VBM分析,比较各组之间各脑区灰质体积的变化。结果:与正常对照组相比,高危组左侧扣带回内部和扣带旁回的灰质体积出现显著性缺损(cluster=26voxel;MNI:-7,-27,30);首发组左侧丘脑灰质体积较小(cluster=580 voxel;MNI:-10,-17,12)。与高危组相比,首发组左侧岛叶灰质体积较小(cluster=1 197 voxel;MNI:-38,-16,-5)。结论:精神分裂症患者大脑结构异常与遗传和环境均有一定的联系。
Objective:By voxel based morphometry( VBM)to compare brain morphological difference between first degree relatives of schizophrenic patients( high-risk group)and negative family history,first-epi-sode,drug-nave patients in schizophrenia and healthy controls. Method:We performed magnetic resonance scanning scans on those meeting the entry criteria for high-risk group(n=10),the first-episode group(n=10) and the normal group( n=10 ),then compared gray matter volume between the groups by VBM. Resuits:Compared with the healthy controls,high-risk group showed significant gray matter volume reduction of the left median cingulate and paracingulate gyri(cluster=26 voxel:MNI:-7,-27,30). Smaller gray matter volume in the left side of the thalamus(cluster=580 voxel:MNI:-10,-17,12)was found in the first-episode group. Compared to high-risk group,first-episode group demonstrated significant gray matter volume reduction of the left insula(cluster=1 197 voxel;MNI:-38,-16,-5). Conciusion:Brain structural abnormalities of schizophrenic patients may be associated with both heredity and environment.
