希氏束起搏引发两种起搏QRS波形1例

摘要 患者男性,56岁,因“胸闷气促5年,心慌数日”就诊。体检:血压89/72mmhg,心率60次/min,律不齐,呼吸19次/min,心界扩大。超声心动图检查示:全心扩大伴左心室收缩功能弥漫性减低,二尖瓣、三尖瓣轻度反流,左心室射血分数(LVEF)26%。常规心电图示窦性心动过缓(52次/min)、不定型心室内阻滞,24h动态心电图示显著的窦性心动过缓伴高度房室传导阻滞,阵发性心房颤动,频发室性期前收缩、室性心动过速及室性逸搏心律。
