剖宫产术后再次开腹7例分析

目的分析剖宫产术后产后出血再次开腹病例,寻找出血原因,总结处理的经验与教训。方法回顾分析7例剖宫产术后因产后出血再次开腹病例,对其一般情况、产检、妊娠合并症、剖宫产手术原因、产后出血情况及原因方面进行分析。结果患者平均孕(3±1)次,产(1±1)次,人流(2±1)次。产检情况:3例有产前检查,4例无产前检查。妊娠合并症:前置胎盘4例,有3例合并妊娠期糖尿病。剖宫产原因:前置胎盘、胎儿宫内窘迫、疤痕子宫、相对性头盆不称。产后出血情况及原因:3例因产后出血宫腔填塞纱条后再次出血;1例产后出血行B-lynch缝合术后再次出血;3例子宫收缩乏力导致剖宫产术后出血。结论产后出血重在预防,在孕前做好宣教,孕期做好产检工作,术中止血迅速彻底及加强术后监测。

Objective Analysis of the cases which postpartum hemorrhage after cesarean section again into the abdomen, looking for bleeding reason, and summarize the experiences and lessons from the treatment.Methods Retrospective analysis 7 cases of the hospital because of postpartum hemorrhage after cesarean section again into the abdomen cases. The general situation, the Maternity check, pregnancy complications, cesarean section surgery causes, postpartum hemorrhage reason analysis. Results （3＋1） times the average pregnancy, childbirth （1＋1） times and abortion （2~：1） times. Maternity check： 3 cases had prenatal care, 4 cases of no prenatal care.Pregnancy complications： placenta previa 4 cases, 4 cases merge GDM. Cesarean section reasons： placenta previa, fetal distress, and scar uterus, relativity head basin don＇t say.Postpartum hemorrhage and reason：3 cases of postpartum hemorrhage bleeding againafter uterine cavity filling yam;1 cases of postpartum hemorrhage bleeding again after B - lynch suture; 3 cases of uterine contraction fatigue cause bleeding after cesarean section. Conclusion Postpartum hemorrhage on prevention, Before becoming pregnant do mission, pregnancy do maternity inspection work; selecting suitable hemostatic method. According to surgical patients and the operation condition; To strengthen the monitoring after operation.
