完全腹腔镜下左肝部分切除术与开放手术的效果比较

Effect Comparison of Complete Laparoscopic Left Partial Hepatectomy with Open Surgery

目的：比较完全腹腔镜下左肝部分切除术与开放手术的效果。方法：选取2012年5月~2014年5月我院收治的80例左肝肿瘤患者,依据手术方式将这些患者分为两组,即腹腔镜手术组（50例）和开放手术组（30例）,运用统计学软件SPSS21.0分析两组患者术中、术后情况、术后并发症、术前术后肝功能指标。分别用率和（x±s）表示计数资料和计量资料,然后分别用χ2和t检验组间比较,检验水准α=0.05。结果：腹腔镜手术组患者的术中出血量（176±59）ml明显比开放手术组（301±119）ml少（t=3.912,P〈0.05）,术后肛门排气时间和住院时间明显比开放手术组短（P〈0.05）,差异有统计学意义。两组患者的手术时间和住院费用之间的差异无统计学意义（P〉0.05）;腹腔镜手术组患者的术后并发症发生率4.0%（2/50）明显比开放手术组33.3%（10/30）低（χ2=5.059,P〈0.05）,差异有统计学意义;术前两组患者的肝功能指标之间的差异均不显著（P〉0.05）;术后1d、4d腹腔镜手术组患者的ALT明显比开放手术组低（t=2.592,P〈0.05）,ALB明显比开放手术组高（t=-7.262,P〈0.05）,差异有统计学意义。但两组患者的TBIL之间的差异均不显著（P〉0.05）。结论：完全腹腔镜下左肝部分切除术比开放手术的效果显著。

Objective： To compare the effect of complete laparoscopic left partial hepatectomy with open surgery.Methods：80 cases of left liver tumors patients in our hospital from May 2012 to May 2014 were selected,these patients were divided into two groups according to surgical methods,namely,laparoscopic surgery group（50 cases） and open surgery group（30 cases）,then the postoperative cases,postoperative complications,preoperative and postoperative liver function of two groups of patients statistically analyzed by software SPSS 21.0.the count data and measurement data were represented by the rate and（x±s）,and then χ2 and t test were used to compare between groups,test level α=0.05.Results：Blood loss in patients with laparoscopic surgery group（176±59）ml was significantly less than the open surgery group（301±119） ml（t=3.912,P〈0.05）,anal exhaust time and hospital stay were significantly higher than the open short-surgery group（P〈0.05）,the differences were statistically significant.But the differences of operative time and hospital costs between the two groups of patients were not significant（P〉0.05）;the incidence of postoperative complications of laparoscopic surgery group 4%（2/50） were significantly lower than the open surgery group 33.3%（10/30）（ χ2=5.059,P〈0.05）,the difference was statistically significant;differences in preoperative liver function between the two groups were not significant（P〉0.05）;postoperative 1d,4d laparoscopic ALT surgery group patients was significantly lower than the open surgery group（t=2.592,P〈0.05）,ALB was significantly higher than the open surgery group（t=-7.262,P〈0.05）,the differences were statistically significant.But the difference of TBIL between the two groups of patients was not significant（P〉0.05）.Conclusion：The effect of complete laparoscopic left partial hepatectomy is more significant than open surgery,so is worthy of promotion in the clinical.
