助产士参与妊娠管理在产妇分娩中的应用效果观察

Observation of Involvement of Midwives in Pregnancy Management on Parturition

目的：探讨助产士参与妊娠管理对产妇分娩及妊娠结局的影响。方法：将2010年2月~2013年10月123例于本院分娩的产妇随机分为对照组（n=61）和观察组（n=62）,分别开展产科传统分娩模式和助产士参与妊娠管理模式,分析两组的妊娠情况（自然分娩率、各阶段及总产程时间）、新生儿一般情况（体重、胎龄、1min和5min Apgar评分）及产后出血和自主排尿情况。结果：观察组的自然分娩率（80.6%vs 55.7%）和自主排尿率（93.5%vs 80.3%）高于对照组（P〈0.05）,产后出血率低于对照组（12.9%vs 27.8%,P〈0.05）,且产后2h、24h出血量均少于对照组（P〈0.05）。观察组第一产程、第二产程及总产程时间均短于对照组,新生儿1min Apgar评分高于对照组（P〈0.05）。结论：助产士参与妊娠管理可提高产妇的自然分娩率,加快产程及促进产后恢复。

Objective： To investigate the effects of involvements of midwives in pregnancy management on parturition and pregnancy outcome.Methods：123 puerpera from February 2010 to October 2013 in our hospital were randomly assigned to receive traditional mode of delivery （control group,n=61） and involvements of midwives in pregnancy management（observation group,n=62）,respectively.The following indices were analyzed between groups on pregnancy （natural birth rate,the various stages of the production process and the total time）,neonatal general （weight,gestational age,1min and 5min Apgar score）,postpartum hemorrhage and independent urination.Results：There were higher rates of natural delivery rate（80.6% vs 55.7%） and self voiding rate （93.5% vs 80.3%） in observation group versus control group,while the rate of postpartum hemorrhage in control group was more than that of observation group （P〈0.05）.The observation group’s hemorrhage volume at 2nd, 24th h after parturition P〈0.05）.The time of first stage,second stage and overall stage in observation group were shorter than in control group （P〈0.05）.Conclusion：The involvement of midwives in pregnancy management is benefit to elevate the natural birth rate,accelerate the production process,reduce postpartum hemorrhage and postpartum promote spontaneous voiding.
