超声辅助定位下中心静脉置管在肿瘤患者的应用体会

Experience of Ultrasound-assisted Positioning Percutaneous Puncture in Central Venous Catheterization of Tumor Patients

目的：比较彩色多普勒超声技术辅助定位下的中心静脉穿刺和传统盲探法穿刺的成功率,探讨一种较佳的深静脉临床置管技术。方法：选择2012年1月~2013年1月我院中晚期肿瘤患者100例,随机分成B、A两组：B组应用超声仪探查右颈内静脉进行辅助定位后穿刺,A组采用盲探法平行颈动脉旁穿刺。比较两种方法的穿刺成功率、失败率和并发症。结果：总成功率B组高于A组,其中B组一次穿刺成功率明显优于A组,具有统计学有意义（P〈0.05）,且无并发症发生。结论：超声辅助的定位下的中心静脉置管技术,简便易行,安全可靠,成功率高,便于示教。

Objective： We performed the study to compare the success rate of ultrasound（US）-assisted positioning percutaneous puncture and traditional percutaneous puncture in central venous catheterization.Methods：From January 2012 to January 2013,one hundred patients with middle or advanced cancer were randomly divided into US-assisted positioning percutaneous catheterization group（group B） and traditional percutaneous catheteration group（group A）.The success rate and complication of puncture were compared.Results：The total success rate of B group was higher than that in group A,And compared with group A,group B has higher one time success rate,And B groups was statistically significant difference between groups（P〈0.05）,and group B have less complications.Conclusion：Ultrasound-assisted positioning percutaneous puncture can significantly improve the success rate of central venous catheteration and decrease the occurrence of complications.The method is effective,safe and reliable,easy to teach.
