河北省临床用血合理性调查分析

Analysis of the Rationality of Clinical Blood Use in Hebei Province

目的调查了解医疗机构临床用血合理性,提出加强管理的建议。方法查阅2013年1月-2014年5月期间河北省二级、三级综合医院的用血住院病历,分析临床用血的合理性。结果河北省三级综合医院临床用血合理性整体好于二级综合医院;各临床科室的冷沉淀、血小板输注较均衡和合理;红细胞合理性欠佳,以妇产科和外科系统最低,分别为79.9%、65.3%;各科室的血浆输注不合理性严重,以内科系统的合理性最低,仅为37.5%;临床用血合理性存在地域的差别,越偏远地区的临床用血合理率越低。结论临床用血存在一定的不合理性,需进一步加强管理和引导。

Objectives To investigate the rationality of clinical blood use in medical institutions, and to put for- ward suggestions for strengthening management. Methods Checking the medical records of the secondary and the tertiary hospital in Hebei province and analyzing the rationality of clinical blood use. Results The rationality of clin- ical blood use of the tertiary hospitals was better than that of the secondary hospitals. The transfusion of cold pre- cipitation and platelet in clinical blood use was balanced and reasonable, while the red blood cell rationality was poor, and the obstetrics and gynecology and surgical system was the lowest （79. 9% and 65.3% respectively）. Plasma transfusion was unreasonable seriously, and the internal medicine system was the lowest （37.5%）. There was difference in the rationality of clinical use blood between different regions, and the reasonable rate of clinical blood use was lower in remote areas. Conclusions There is a certain irrationality in clinical blood use, and the management and guidance should be further strengthened.
