降钙素原与C反应蛋白水平检测在新生儿病理性黄疸中的应用价值 被引量:8

Application Value of Neonatal Pathological Jaundice in the Procalcitonin and C Reactive Protein Levels

摘要 目的探讨降钙素原(PCT)与C-反应蛋白(CRP)水平检测在新生儿病理性黄疸(PNJ)中的应用价值。方法选取2011年1月至2013年12月细菌性感染PNJ新生儿72例,将其依据细菌检验结果分为重症组(30例)和局部组(42例),并选取同期鉴定为非细菌性感染的PNJ新生儿72例作为对照组;比较3组新生儿CRP、PCT和血清总胆红素(TSB)水平。并将72例细菌性感染的PNJ新生儿分为两组,指导组根据PCT水平指导治疗,而未指导组仅根据经验和症状进行治疗。结果局部组新生儿CRP、PCT和TSB水平明显高于对照组,且重症组新生儿CRP、PCT和TSB水平明显高于局部组,差异均有统计学意义(均P<0.05);指导组新生儿住院时间和住院费用均明显低于未指导组,差异有统计学意义(P<0.05)。结论 CRP和PCT是细菌性感染PNJ可靠的诊断指标,对于PNJ的临床治疗具有重要的指导价值。 Objective To investigate the pathological jaundice of newborn(PNJ)in the serum procalcitonin (PCT)and C reactive protein(CRP)level of the application value of detection.Methods From 2011 January to 2013 December in our hospital for treatment of 72 cases of neonatal bacterial infection of PNJ,the test results on the basis of the bacteria are divided into 30 cases in severe group and the local group of 42 cases were selected,and were identified as PNJ with non bacterial infection in 72 cases as the control group; the total bilirubin of newborn CRP, PCT and serum(TSB)level between the 3 groups.And PNJ in 72 cases of neonates with bacterial infection were divided into two groups according to the level of PCT, the steering group to guide treatment,without guidance group was treated according to the experience and symptoms.Results The local group of neonatal CRP, PCT and TSB levels were significantly higher than the control group,and severe group of CRP, PCT and TSB levels were significantly higher than that of the local group,the differences were statistically significant(P-〈0.05);group of neonatal hospitalization time and hospitalization costs were significantly lower than those without guidance group, the difference was statistically significant(P〈0.05).Conclusiorl CRP and PCT are reliable index for diagnosis of PNJ infection,which has an important guiding value for clinical treatment of PNJ.
