右美托咪定在隆鼻术中的应用

Application of Dexmedetomidine to augmentation rhinoplasty

目的：探讨右美托咪定在隆鼻术中应用的安全性与有效性。方法择期行隆鼻手术患者60例，随机等分为两组。静脉麻醉诱导后行局部麻醉。麻醉维持：D组应用0.5μg/（kg·h）右美托咪定至术毕前10 min，P组应用4 mg/（kg·h）丙泊酚至术毕。结果两组T0～T3的OAA/S评分和体动反应差异无统计学意义。与P组比较，D组辅助呼吸次数、芬太尼诱导时呛咳发生率降低，心动过缓发生率高（P＜0.05）。结论右美托咪定用于局部麻醉下行隆鼻手术的术中镇静能够达到丙泊酚的镇静强度，且安全性高。

Objective To explore the feasibility and safety of the application of dexm edetom idine to augm entation rhinoplasty.Methods 60 cases choosing to undergo augm entation rhinoplasty were random ly divided into 2 groups.Lo-cal anaesthesia was conducted after vein anesthesia induction .Maintenance of anesthesia：D exm edetom idine was infused at a rate of 0.5 μg/（kg·h）in group D until 10 m inutes before the end of surgery.Propofol was infused at a rate of 4 mg/（kg·h）in group P untilthe end ofsurgery.Results OAA/S score atT0~T3 and the body movementreaction in two groups had no difference of statistical significance.C om pared with group P ,assisted respiration frequency and fen-tanyl-induced cough occurrence rate in group D were low;bradycardia incidence in group D was high （P〈0 .05 ）. Con clusion D exm edetom idine for sedation under local anesthesia in augm entation rhinoplasty can achieve the strength of propofol sedation with high security.
