健康管理与临床治疗相结合对高血压病患者生活质量的影响研究

Effect of health management combined with clinical treatment on the quality of life in patients with hypertension

目的：评估健康管理与临床治疗相结合对高血压病患者生活质量的影响。方法：将316例符合条件且排除继发因素的高血压病2-3级患者随机分为观察组和对照组,2组患者的临床药物治疗和一般护理方法相同,观察组158例患者在定期接受专家门诊药物治疗的同时接受专人健康管理,对照组仅定期接受专家门诊药物治疗。所有患者均在首次专家门诊就诊时留下有效的联系方式,签署知情同意书,在首诊和首诊后24个月时分别接受生活质量量表（SF-36健康量表）测试和健康知识问卷调查,专人定期随访,了解住院和生活质量情况,同时对患者血压、治疗依从性及并发症情况进行追踪和效果评价。结果：干预24个月后2组患者健康知识得分比较差异有统计学意义;2组的生活质量生理功能、生理职能、社会功能、躯体疼痛、情感职能、活力、总体健康、精神健康评价及与其观察前比较等多项指标差异有统计学意义;观察组血压控制达标率和药物治疗依从性均优于对照组,并且观察组脑卒中冠心病等并发症的发生率明显低于对照组。结论：实施健康管理与临床治疗相结合对改善高血压病患者的生活质量、降低其血压水平、提高其药物治疗的依从性及降低并发症的发生等有积极的作用。

Objective：To evaluate the effect on the quality of life of hypertension patients who were treaded with health management and clinical treatment.Method：316cases of 2-3hypertension patients who were eligible and excluded secondary hypertension were divided into 2groups.2groups of patients had clinical drug treatment and the same general nursing method.158 patients in observation group accepted regular expert outpatient service drugs treatment,they accepted the specialist health management at the same time.The control group accepted regular expert outpatient service drugs treatment only.All patients left effective contact information in the specialist clinics for the first time,sign the informed consent,accepted the quality of life scale（SF-36 health scale）test and health knowledge questionnaire in the first option at 24 months after first option.The specialist followed-up regularly to understand the situation in hospital and quality of life,at the same time track the patients＇ blood pressure、treatment adherence and complications.Result：After 24 months,the knowledge score of 2groups had extremely significant differences.Physiological functions,physical function,social function,body pain,emotional functions,dynamic,overall health,mental health evaluation also had extremely significant difference compared with the level of before.Blood pressure control success rate and blood pressure medication adherence of the observation group were superior to control group,and the incidence of coronary heart disease,stroke and other complications were lower than that of the control group.Conclusion：The health management combined with clinical treatment improved the quality of life in hypertension patients,lowered the level of their blood pressure,improved the medication adherence and reduced the occurrence of complications.
