摘要
目的 99 Tcm-MNLS、99 Tcm-HL91放射性核素乏氧显像监测肿瘤放疗后乏氧状态变化的对比研究。方法建立24只荷瘤小鼠模型,待肿瘤直径约1cm时,按随机数字表法等分为99 Tcm-HL91即刻显像组、99 Tcm-HL91 48h显像组、99 Tcm-MNLS即刻显像组、99 Tcm-MNLS 48h显像组。荷瘤小鼠肿瘤部位均给予单次25Gy的放射治疗,分别于放疗后即刻、48h注射相应乏氧显像剂99 Tcm-MNLS或99 Tcm-HL91,2h后显像,应用感兴趣区技术(ROI)计算肿瘤/非肿瘤组织放射性比值(T/NT),肿瘤标本行乏氧诱导因子-1α(HIF-1α)免疫组织化学染色。对所得T/NT比值及HIF-1α免疫组织化学结果进行分析。结果99 TcmMNLS即刻显像组T/NT比值、HIF-1α表达量(3.36±0.20、88.55%±0.83%)均较其48h显像组(2.25±0.43、22.75%±3.35%)增高,差异均有统计学意义(P<0.05)。99 Tcm-HL91组即刻显像组T/NT比值、HIF-1α表达量(3.23±0.90、90.55%±2.61%)均较其48h显像组(2.79±0.85、25.88%±3.76%)增高,差异均有统计学意义(P<0.05)。99 Tcm-MNLS和99 Tcm-HL91组即刻显像组T/NT、HIF-1α表达量差异均无统计学意义(P>0.05),48h显像组T/NT差异均无统计学意义(P>0.05)。99 TcmMNLS显像在腹部除肾脏外无明显浓集,而99 Tcm-HL91全腹均有浓集。结论 99 Tcm-MNLS与99 Tcm-HL91乏氧显像监测肿瘤放疗后乏氧状态的变化效能相近,均能准确地反映肿瘤内的乏氧状态。
Objective To compare the imaging of ^99Tc^m MNLS and ^99Tc^m-HL91 for the hypoxic assessment in the tumor af- ter radiotherapy. Methods Twenty four Kunming mice models bearing H22 liver cancer xenografts which tumor were treated ra- diotherapy for 25 Gy were divided into 99Tcm-MNLS instant group,^99Tc^m-MNLS 48 h group,^99Tc^m-HL91 instant group,^99Tc^m- HLgl 48 h group and injected the matching ^99Tc^m-MNLS or ^99Tc^m-HL9I to image. The technique of ROI was adopted to calculated the T/NT on the image. Immunohistochemical stain methods were used to evaluate the level of HIF-1α about tumors after imaging. Two sample t test was performed. Results The T/NT and the HIF-1α level about instant (3.36±0.20,88.55 ±0.83 % )were higher than and 48 h (2.25±0.43,22.75%±3.35%)in ^99Tc^m-MNLS group(P〈0.05) ,then in ^99Tc^m-HL91 group the T/NT and the HIF-1α level about instant (3.23 ± 0. 90,90.55 %± 2. 61%) were higher than 48 h (2.79 ± 0. 85,25.88 ± 3. 76 %) (P〈0.05). The difference of T/NT, HIF-1α level about instant in ^99Tc^m-MNLS group and ^99Tc^m-HL91 group were no significant(P〉 0.05)and the same to the T/NT,HIF-1α level about 48h in ^99Tc^m-MNLS group and ^99Tc^m-HL91 group(P〉0. 05). Conclusion ^99Tc^m-MNLS hypoxia imaging could provide the change of hypoxia comparable to ^99Tc^m-HL91, and the two radiopharmaceutical could both show high accuracy rate in evaluating the hypoxia in the tumor.
