超声斑点追踪成像技术评价系统性红斑狼疮左室功能的临床研究

摘要 目的探讨超声斑点追踪成像技术(STI)在评价系统性红斑狼疮(SLE)患者左室功能中的价值。方法选择50例女性SLE患者为研究组,30名健康女性为对照组,分别行超声心动图检查,获取标准心尖四腔、三腔、两腔和短轴二尖瓣、乳头肌、心尖水平图图像,分别测量和比较研究组和对照组各节段的纵向、径向和圆周的收缩期峰值应变,对数值进行统计学研究。结果对照组左室长轴纵向应变在心尖部最大,与本组同一心室水平的基底部和中间的纵向应变值相比,差异有统计学意义(P<0.05);短轴径向应变差异无统计学意义(P>0.05);圆周应变则仅在前间隔和后间隔的基底及中间段高于同水平心尖段(P<0.05)。与对照组相比,研究组所有心室水平不同节段的纵向、短轴径向及圆周应变值均降低(P<0.05)。结论 STI能较好地评价早期SLE患者左室功能。 Objective To investigate the value of speckle tracking imaging (STI) in evaluation of left ventricular func-tion of patients with systemic lupus erythematosus (SLE). Methods 50 female patients with SLE were selelcted as study group, and 30 healthy female were selelcted as control group. Two groups were all given echocardiography examination. The standard apical four chamber, three chamber, two chamber and short axis mitral valve, papillary muscle, apical lev-el images were get. The longitudinal, radial and circumferential systolic peak strain of each section in the study and control groups were detected and compared. The results were analyzed statistically. Results Control group showed that left ventricular long-axis longitudinal strain was the largest in the apex of heart, compared with longitudinal strain of pars basilaris and middle section in the same ventricle level, the differences were statistically significant (P〈0.05);short axial-radial strain had no statistically significant difference (P〉0.05);circumferential strain in anteroseptum and post-septal of the basal and middle section were higher than same levels of apical segment (P〈 0.05). Compared with the control group, the longitudinal, short axis-radial and circumferential strain values of different segments of ventricle all reduced in the study group (P 〈 0.05). Conclusion STI can be used to assess the left ventricular function in early phase SLE patients.
