比较传统开放手术与腹腔镜手术治疗肾盂输尿管结石的临床效果

目的比较传统开放手术与腹腔镜手术在肾盂输尿管结石患者中应用的临床效果。方法收集我院从2013年5月至2014年6月确诊为肾盂输尿管结石患者100例,采用随机数字表法将100例肾盂输尿管结石患者分为观察组(50例)和对照组(50例);观察组采取腹腔镜手术治疗,对照组采取传统开放手术治疗,观察对比两组患者的术中出血量、住院时间,术后恢复情况及术后并发症情况。结果观察组术中出平均血量为12.56±6.32ml、住院时间为13.51±3.52天,对照组术中出平均血量为53.12±31.57ml,住院时间为18.13±4.96天,两组比较具有统计学意义(P<0.05);观察组术后肠功能恢复时间为1.26±0.82天,下床活动时间1.35±0.52天,对照组术后肠功能恢复时间为2.61±0.76天,下床活动时间2.96±1.06天,两组比较差异具有统计学意义(P<0.05);术后并发症主要有尿瘘与感染,观察组的并发症与对照组相当。结论腹腔镜手术较传统开放手术具有创伤小、术中出血量少、术后恢复时间快等优点,且并发症与传统开放手术相当,因此为患者的早期恢复提供了良好的条件,为患者的生活提供了方便,减轻了患者痛苦,值得在临床进一步推广应用。

Objectives Analysis and comparison of the traditional open operation and clinical effect of laparoscopic operation in pelvic ureteral calculi in patients with. Methods Collected in our hospital from 2013 May to 2014 June, 100 cases were diagnosed renal pelvic and ureteral calculi patients, randomly 100 cases of renal pelvic and ureteral calculi patients were divided into observa- tion group （50 cases） and control group （50 cases） ; observation group adopt laparoscopie operation for the treatment, control group took the traditional open operation for the treatment of bleeding, quantity, time of hospitalization were compared between two groups of patients with complications after surgery, and the recovery of postoperative. Results The patients in the observation group than the average blood was 12. 56 ＋ 6. 32ml, the time of hospitalization for 13.51 ＋ 3.52 d, the control group was a mean blood was 53. 12 4-31.57ml, the time of hospitalization for 18. 13 ＋ 4. 96 d, the two groups was statistically significant （P 〈 0.05 ） ; observation group postoperative intestinal function recovery time was 1.26 ＋ 0. 82d, time to ambulation l. 35 ＋ 0. 52d, the control group of postoperative intestinal function recovery＂ time was 2. 61 ＋ 0. 76d, time to ambulation 2. 96 ＋ 1.06d, with statistically significant difference between the two groups （ P 〈 0. 05 ） ; the main postoperative complications a fistula and infection, the observation group and the control group of considerable complications. Conclusions Laparoseopic surgery than traditional open surgery with small trauma, less bleeding, postoperative recovery time quickly and complications with traditional open surgery is quite, therefore provides good conditions for patients with early recovery, and provides convenience for the life of patients, reduce the pain of patients, is worthy of further popularization and application in clinical.
