摘要 重症急性胰腺炎(sever acute pancreatitis,SAP)在起病初期,即有由细胞因子介导的全身炎症反应综合征(systematic inflammatory response syndrome,SIRS)引起的多器官功能衰竭(multiple organ dysfunction failure,MOF)。SIRS和MOF是危及患者生命的主要并发症,其特点是多器官功能不全,其中急性肺功能不全合并MOF是最常见的、也是发病率最高的胰腺外器官。临床上表现为急性呼吸窘迫症,病情危急,严重危及生命。因此,研究SAP并发MOF的机制可为SAP治疗提高理论基础,本文就近年来国内外有关SAP合并急性肺损伤的机制进行综述。 During the early stage of severe pancreatitis( SAP),multiple organ failure( MOF) is occurred owing to cytokine-mediated systemic inflammatory response syndrome( SIRS). SIRS and MOF are the major lifethreatening complications,which is characterized by multiple organ dysfunctions such as pulmonary insufficiency,cardiovascular dysfunction,renal insufficiency and acute pulmonary insufficiency. Moreover,acute pulmonary insufficiency is the most common complication of severe acute pancreatic manifestations,characterized by a serious life-threatening acute respiratory distress syndrome. This paper will review recent development of the severe acute pancreatitis with pulmonary damages.
