采用临床路径模式管理老年性白内障医疗过程的效果观察

To observe the clinical path management model of senile cataract medical effect

目的探讨临床路径模式在管理老年性白内障医疗过程中的应用价值。方法选自2012年1月至2013年1月在山东省胶州市人民医院眼科就医的老年性白内障患者84例(84只眼)行超声乳化白内障吸除联合后房型人工晶状体植入术治疗,采用数字表法随机分为研究组和对照组,每组各42例(42只眼),研究组的医疗流程实施临床路径模式管理,对照组遵循传统模式的常规进行实施医疗流程。比较研究组和对照组两组患者的平均住院天数、住院费用及药费情况,采用t检验进行统计学分析。结果研究组和对照组两组患者的医疗情况,研究组的平均住院天数为(3.50±0.62)d、住院费用为(3 421±286.07)元、药费为(406.29±29.16)元;对照组的平均住院天数为(4.98±1.29)d、住院费用为(3 945.86±271.06)元、药费为(586.29±52.38)元。结果表明研究组患者的平均住院天数、住院费用及药费均低于对照组,差异有统计学意义(t=6.701 4,8.631 2,19.458 5;P<0.05)。结论临床路径模式应用于老年性白内障医疗过程的管理中,可有效缩短患者的住院天数、减少住院费用及用药费用。可以促进医院资源的合理利用和医疗质量,提高医院运转效率,减轻患者的医疗负担,值得临床推广和应用。

Objective To explore the application value of clinical path model in the management of senile cataract in medical treatment.Methods 84 cases （ 84 eyes ） with senile cataract take medical treatment in People′s hospital of Jiaozhou city Shandong province department of ophthalmology from 2012 January to 2013 January who underwent phacoemulsification and posterior chamber intraocular lens implantation.Randomly divided into study group and control group, each group had 42 cases （ 42 eyes ） . Study group of medical process get management mode of clinical path, the control group followed the traditional model of the routine medical procedures .Patients were compared between study group and control group in mean length of hospital stay,hospitalization expenses and expenses for medicine.T test was used for statistical analysis.Results There are study group and the control group of two patients with medical conditions：study group,the average hospital stay was （3.50 ±0.62） d,hospitalization expenses is （3 421± 286.07）Yuan,prescription charge is （406.29±29.16）Yuan;control group,the average hospital stay was （4.98±1.29）d,hospitalization expenses is （3 945.86±271.06）Yuan,prescription charge is （586.29± 52.38）Yuan.The results showed that the average hospitalization days,cost of hospitalization and prescription charge in study group is lower than the control group.The difference was statistically significant （ t=6.701 4, 8.631 2,19.458 5;P ＆lt;0.05）.Conclusion Application of clinical path in senile cataract medical process management can effectively reduce the hospitalization days,cost of hospitalization and prescription charge.It can be promote the rational use of hospital resources, improve the medical quality and hospital running efficiency,reduced the medical burden of patients.It is worthy of clinical promotion and application.
