光学相干断层扫描检测前房角角度与三面镜检查前房角分级的对应性研究 被引量:1

摘要 目的:探讨光学相干断层扫描( OCT)检测前房角角度与三面镜检查前房角分级的对应性关系。方法收集2013年1月至2013年6月就诊于山西省临汾市尧都区眼科医院门诊的患者32例(64眼),双眼均纳入研究,排除有角膜水肿或混浊、角膜炎症、角膜斑翳、内眼手术史及眼球穿通伤病史的患者。所有受试者均接受裂隙灯显微镜下三面镜检查前房角分级及OCT检测前房角角度。采用三面镜检查并记录受试者患眼的眼别及鼻侧、颞侧、上方及下方象限的前房角分级情况(采用Scheie分级法);采用OCT检测并记录受试者患眼的眼别及鼻侧、颞侧、上方及下方象限的前房角角度。结果受试者32例(64只眼,256个象限)。其中,男性7例(14只眼,56个象限),占21.88%;女性25例(50只眼,200个象限),占78.12%。年龄26~69岁,平均45.4岁。采用三面镜检查受试者前房角分级与采用光学相干断层扫描检测前房角角度的结果情况,分别为:宽角,鼻侧为43.12°±6.21°,颞侧为46.23°±7.01°,上方为40.12°±5.66°,下方为45.17°±6.13°,前房角角度范围为35°~50°;窄Ⅰ,鼻侧为34.15°±4.17°,颞侧为35.13°±3.13°,上方为33.10°±4.17°,下方为37.10°±3.07°,前房角角度范围为30°~40°;窄Ⅱ,鼻侧为24.88°±2.49°,颞侧为24.96°±4.06°,上方为23.05°±3.86°,下方为25.03°±3.01°,前房角角度范围为20°~30°;窄Ⅲ,鼻侧为15.23°±5.38°,颞侧为15.05°±6.01°,上方为13.71°±5.26°,下方为14.39°±5.29°,前房角角度范围为10°~20°;窄Ⅳ,鼻侧为5.10°±4.11°,颞侧为5.31°±3.21°,上方为4.67°±4.07°,下方为5.47°±2.87°,前房角角度范围为0°~10°。结论采用OCT检测前房角角度可以作为前房角分级检查的新方法。 Objective To explore of correspondence on examination the degree of anterior chamber angle by optical coherence tomography( OCT) and the grade of anterior chamber angle by three-mirror lens. Methods 32 outpatient(64 eyes) were included in the analysis from January to June in 2013 in Linfen Yaodu Eye Hospital.Excluding any patient with corneal edema and opacity, keratitis, corneal macula, intraocular surgery and penetrating injury history.All subjects examined the grade of anterior chamber angle by three-mirror lens under slit lamp and the degree of anterior chamber angle by OCT.Examine and record the tested eye and the grade of nasal,temporal,superior,inferior anterior chamber angle by Scheie′s method. Examine and record the degree of nasal,temporal,superior,inferior anterior chamber angle by OCT.Results 32 Case ( 64 Eye,256 Quadrants) were included.21.88% of them were male (7 cases ,14 eyes,56 quadrants) and 78.12%of them were female (25 patients,50 eyes,200 quadrants).The age of participants were from 26 to 69 years old,and the average age was 45.4.The results of the grade of anterior chamber angle examined by three-mirror lens and the degree of anterior chamber angle examined by OCT were following blow:wide angle,nasal side was 43.12°±6.21°,temporal side was 46.23°±7.01°,superior side was 40.12°±5.66°and inferior side was 45.17°±6.13°,the anterior chamber angle was in the range of 35°-50°.NarrowⅠ,nasal side was 34.15°±4.17°,temporal side was 35.13°±3.13°,superior side was 33.10°± 4.17°and inferior side was 37.10°±3.07°,the anterior chamber angle was in the range of 30°-40°.Narrow II,nasal side was 24.88°±2.49°,temporal side was 24.96°±4.06°,superior side was 23.05°±3.86°and inferior side was 25.03°±3.01°,the anterior chamber angle was in the range of 20°-30°.Narrow III,nasal side was 15.23°±5.38°,temporal side was 15.05°±6.01°,superior side was 13.71°±5.26°and inferior side was 14.39°±5.29°,the anterior chamber angle was in the range of 10°-20°;Narrow IV,nasal side was 5.10°±4.11°,temporal side was 5.31°±3.21°,superior side was 4.67°±4.07°and inferior side was 5.47° ±2.87°,the anterior chamber angle was in the range of0 °1-0 °.Conclusion OCT can be used as a new method for examining the grade of interior chamber angle.
