动态动脉硬化指数、hs-CRP与原发性高血压患者冠心病相关性分析 被引量:3

摘要 目的探讨动态动脉硬化指数(AASI)、hs-CRP与原发性高血压患者冠心病之间的相关性。方法选用我院住院部高血压患者190例,根据24 h动态血压(ABPM)监测结果计算AASI,冠状动脉造影结果进行Gensini评分,根据评分结果:0-1分为非冠心病组(n=78),2-31分为轻、中度冠脉病变组(n=74),≥32分为重度冠脉病变组(n=38),采用单因素和Logistic回归分析与冠脉相关的危险因素。结果非冠心病组年龄、AASI、hs-CRP、24h HR、非杓型血压与冠心病组比较差异有统计学意义(P<0.05);在多因素logistic回归分析中,AASI和hs-CRP是影响冠心病发生的独立危险因素;3组AASI、hs-CRP组间比较差异有统计学意义(FAASI=35.9,P<0.05;Fhs-CRP=27.47,P<0.05)。重度冠脉病变组AASI、hs-CRP水平高于轻、中度组以及非冠心病组(P<0.05)。结论 AASI,hs-CRP对高血压患者出现冠脉病变及其严重程度有较好的预测价值。 Objective To explore the correlation between the AASI,hs-CRP and coronary heart disease in primary hypertensive patients. Methods A total of 190 consecutive patients with primary hypertension which were hospitalized in our department were enrolled in this study. AASI was calculated based on the 24h groups based on the ABPM, and all patients were divided into three groups according to the Gensini grade result. : non CHD group(0-1 score, n=78}, mild lesion(2-31 score, n=74) and severe lesion( ≥32 score, n = 38 ) group. Correlative risk factors were analyzed with One-ANOVA and multivariate logistic re- gression. Results There was statistical meaning comparing the age, AASI, hs-CRP, 24h HR, non-dipper type hypertension between the non-CHD group and CHD group ( P〈0.05 ) ; and the AASI, hs-CRP was still risk factors contributing to the CHD; One-way ANOVA showed AASI, hs-CRP levels was significantly dif- ferent among the three groups ( FAASI = 35. 9, P〈0.05 ; Fhs_cRP = 27. 47, P〈0.05 ) , furthermore, Bonferroni multi-comparison demonstrated the level of the AASI, hs-CRP in the severe group was more higher than that in the non-CHD group and mild group (P〈0.05). Conclusion AASI and hs-CRP is closely related to the CHD and the severity of coronary artery stenosis.
