摘要
目的探讨在团队管理下高血压治疗的效果。方法选取2013年1月至2014年8月在我院高血压门诊就诊的高血压人群80例,接受团队管理的40例患者,未接受团队管理患者40例。进行团队管理的高血压人群作为团队管理组,给予患者(1)进行生活方式干预:限盐、戒烟、控制饮酒、控制体重、加强运动;(2)定期测量血压,调整药物;(3)按时提醒服药。未进行管理的40例高血压患者为普通高血压组作为对照。回顾性分析两组人群在改善生活方式(戒烟、限酒、体重指数、运动)、治疗率、高血压控制率、心脑血管病发生率等。对团队管理前后的疗效进行比较,将所有问卷建立数据库比较分析,计数资料用,检验,计量资料用t检验。结果①团队管理组不良生活方式较普通高血压组明显改善[5%(2/40)比20%(8/40),x^2=4.114,P〈0.05];其中吸烟[0(0/40)比12.5%(5/40),饮酒(0比5%(2/20)],体重指数[5%(2/40)比7.5%(3/40)]。②团队管理组血压治疗率高于普通高血压组[100%(40/40)比80%(32/40),x^2=6.806,P〈0.05]。③团队管理组血压下降水平高于普通高血压组[收缩压:(136±7)mmHg比(144±11)mmHg,t=-4.306,P〈0.05;舒张压:(81±6)比(89±7)mmH,t=-5.303,P〈0.05]。④团队管理组血压控制率高于普通组[80%(32/40)比45%(18/40),t=10.453,P〈0.05]。⑤团队管理组心脑血管疾病发生率低于普通高血压组[5%(2/40)比10%(4/40)]。⑥高血压患者在团队管理前后疗效比较:不良生活方式人次明显减低[35%(14/40)比5%(2/40),P〈0.05];高血压治疗率升高[65%(26/40)比100%(40/40),P〈0.05];血压明显下降[收缩压:(153±11)mmHg比(136±7)mmHg,P〈0.05;舒张压(96±8)mmHg比(81±6)mmHg,P〈0.05];无新发心血管疾病。结论规范高血压人群的团队管理,可以改善高血压人群的生活方式,有效控制血压,降低心脑血管疾病的发生。
Objective To investigate the clinical effect of team management of hypertension. Methods The team management group included 40 hypertensive patients with team management: (1) Giving lifestyle interventions to patients: salt restrictions, quit-smoking, limit alcohol, weight control, moderate exercise. ( 2 ) Regular blood pressure measurement, adjustment of drug. ( 3 ) Reminded medication on time. The common hypertension group included 40 hypertensive cases without team management. A retrospective analysis of two groups evaluated improvement of lifestyle, treatment rate, control rate of hypertension, cardiovascular and cerebrovascular events incidence rate and so on. Furthermore, a comparative efficacy analysis before and after team management was carried out within team management group. Results ( 1 ) Lifestyle was improved significantly in team management group compared with common hypertension group [5% (2/40) vs 20% (8/40) ,x^2 =4. 114,P 〈0.05] ; (2) Compared with common hypertension group, blood pressure treatment rate was higher in team management group [ 100% (40/40) vs 80% (32/40) ,x^2 = 6. 806 ,P 〈 0.05] ; (3) Blood pressure decreasement was more distinct in team management group than common hypertension group (Systolic blood pressrue: [ (136 ±7 ) vs( 144 ± 11 ) mmHg, t = - 4. 306,P 〈 0.05 ] ; Diastolic blood pressure : [ ( 81 ±6 ) vs ( 89 ±7 ) mmHg, t = - 5. 303, P 〈 0.05 ] ; (4) Compared with common hypertension group, blood pressure control rate was higher in team management group [ 80% (32/40) vs 45% ( 18/40 ) , t = 10. 453, P 〈 0.05 ] ; ( 5 ) Cardiovascular disease incidence rate was lower in team management group [5% (2/40) vs 10% (4/40)1. (6) After team management within hypertension patients, lifestyle of patients was improved and the treatment rate was increased [ 65 % (26/40) vs 100% (40/40), P 〈 0.05 ], the level of blood pressure decreasement was more distinct[ Systolic blood pressrue: ( 153 ± 11 ) mmHg vs ( 136 ±7 ) mmHg,P 〈 0.05 ; Diastolic blood pressure (96 ± 8 )mmHg vs (81 ± 6)mmHg,P 〈 0.05 ]. the blood pressure control rate was increased [ 20% (8/40) vs 80% (32/40) , P 〈 0. 05 ]. Furthermore, no cardiocerebral vascular diseases oecured. Coneluslon Standard team management of hypertension could improve patients' lifestyle and blood pressure control rate, reduce the incidence of cardiovascular and cerebrovascular diseases, additionally enhance the quality of life.
