经胸肌入路锁骨下淋巴结清扫术治疗乳腺癌效果观察

Observation on Curative Effects of Infraclavicula Lymph Node Dissection Via Pectoralis Approach in the Treatment of Breast Cancer

目的:探讨经胸肌入路锁骨下淋巴结清扫术治疗乳腺癌效果。方法:本组86例患者根据随机数字表法分为观察组和对照组,每组43例。对照组采用常规电刀手术治疗;观察组采用经胸肌入路锁骨下淋巴结清扫术治疗。对比分析两组患者手术时间、引流量、术中出血量及并发症发生情况。结果:观察组手术时间显著短于对照组,引流量显著多于对照组,术中出血量显著少于对照组,差异均有统计学意义(P<0.05);观察组并发症发生率显著低于对照组,差异有统计学意义(P<0.05)。结论:对乳腺癌患者在Auchincloss术基础上经胸肌入路锁骨下淋巴结清扫术,淋巴结清除彻底,手术时间短,引流量多,术中出血量少,且并发症少。

Objective：To explore the curative effects of infraclavicula lymph node dissection via pectoralis approach in the treatment of breast cancer.Method：86 cases were divided into the observation group（n=43） and the control group（n=43） according to the random number table.The control group was given conventional surgery with electric scalpel,while the observation group was given infraclavicula lymph node dissection via pectoralis approach.The two groups of patients were compared for operation time,volume of drainage,intraoperative blood loss and complications.Result：The observation group had significantly shorter operation time,more volume of drainage and less intraoperative blood loss than the control group,with statistical differences（P〈0.05）;and the incidence of complications in the observation group was significantly lower than the control group,with statistical difference（P〈0.05）.Conclusion：For patients with breast cancer,infraclavicula lymph node dissection via pectoralis approach combined with Auchincloss can thoroughly remove the lymph nodes with shorter operation time,more volume of drainage,less intraoperative blood loss and fewer complications.
