人工股骨头与全髋关节置换术治疗老年股骨颈骨折的疗效对比

Efficacy Comparison of the Artificial Thigh Bone Replacement and Total Hip Replacement in the Treatment of Femoral Neck Fractures

目的:比较人工股骨头置换术与全髋关节置换术两种手术方案治疗老年人股骨颈骨折的治疗效果。方法:回顾性分析笔者所在医院收治的120例老年股骨颈骨折患者的资料,按照手术方式分为试验组和对照组,试验组采取全髋关节置换术,对照组采取人工股骨头置换术,比较两组的治疗效果。结果:试验组患者的手术时间长于对照组,术中出血量多于对照组,差异有统计学意义(P<0.05);试验组患者术后0.5、1、3、5年的Harris评分均明显高于对照组患者,差异有统计学意义(P<0.05)。结论:全髋关节置换术在术后近期及远期的髋关节功能恢复上明显优于人工股骨头置换术,适用于对髋关节功能要求高、全身状况较好的老年患者。

Objective：To compare the effect of artificial thigh bone replacement and total hip replacement in the treatment of elderly femoral neck fractures.Method：120 cases of elderly femoral neck fractures patients with information in our hospital were retrospective analyzed,according to surgical method they were divided into the experimental group and the control group,the experimental group was taken total hip replacement,the control group was taken artificial thigh bone replacement,the effects of the two groups were compared.Result：Operative time of the experimental group was longer than the control group,blood loss was more than the control group as well,the differences were statistically significant（P〈0.05）;Harris score of the experimental group after six months,one year,three years,five years were significantly higher than those of the control group,the differences between groups were statistically significant（P〈0.05）.Conclusion：Total hip replacement in short and long term postoperative functional recovery of the hip are significantly better than artificial thigh bone replacement,which is applicable for hip function requirements,elderly patients with good general condition.
