限制性液体复苏在产科失血性休克治疗中的应用

The Application of Limited Fluid Resuscitation in the Treatment of Hemorrhagic Shock in Obstetrics

目的:探讨限制性液体复苏在产科失血性休克治疗中的应用价值。方法:选取笔者所在医院产科近5年间收治的失血性休克患者90例,采用随机数字分组法将上述患者分为观察组和对照组。观察组采用限制性液体复苏法进行治疗,对照组采用常规液体复苏法进行治疗。结果:观察组HGB、HCT、PC、PT、APTT、BE、血压值均优于对照组,差异有统计学意义(P<0.05);两组患者输液量、出血量、DIC、ARDS及病死率的发生率比较差异有统计学意义(P<0.05);与对照组相比,观察组患者治疗满意度较高,差异有统计学意义(字2=8.78,P<0.01)。结论:采用限制性液体复苏法保证了失血性休克患者组织器官的血液及氧气供应,降低患者并发症的发生率及死亡率,具有临床应用推广价值。

Objective：To explore the application value of limited fluid resuscitation in the treatment of hemorrhagic shock in obstetrics.Method：90 cases of hemorrhagic shock in our hospital obstetrics in recent five years were selected,they were randomly divided into the observation group and the control group.The observation group was given limited fluid resuscitation method treatment,the control group was taken the conventional fluid resuscitation method treatment.Result：The HGB,HCT,PC,PT,APTT,BE,blood pressure values in the observation group were better than those in the control group,the differences were statistical significance（P〈0.05）;the two groups of patients in the infusion volume,bleeding volume,DIC,ARDS and mortality incidence had statistical significance（P〈0.05）;the satisfaction in the observation group was higher than that in the control group（ 字2=8.78,P〈0.01）.Conclusion：The use of limited fluid resuscitation can ensure the blood and oxygen supply to tissues and organs of hemorrhagic shock patients,decrease the occurrence rate of complications and death rate,it has clinical application and popularization value.
