非糖尿病性巨大儿发生的相关因素及妊娠结局分析

Analysis in relevant factors of development of non-diabetes macrosomia and their influence on pregnancy prognosis

目的探讨非糖尿病性巨大儿发生的相关因素及对母婴妊娠结局的影响。方法回顾性分析2013年2月—2014年2月分娩的122例巨大儿(观察组)的临床资料,并与同期分娩正常新生儿150例(对照组)比较。结果观察组孕妇年龄、体质量指数(BMI)、孕期体质量增加、孕周均显著大于对照组(P<0.05或P<0.01),经产妇比例高于对照组(P<0.01);2组在孕产妇宫高、腹围、胎儿双顶径(BPD)和股骨长度(FL)差异均具有统计学意义(P<0.01);妊娠结局显示,2组在产程延长、分娩方式、软产道损伤、产后出血方面,差异有统计学意义(P<0.05或P<0.01)。结论非糖尿病性巨大儿发生与孕妇年龄、BMI、孕期体质量增加、孕周和产次等有关,积极开展干预措施对降低巨大儿发生率,提高母婴健康水平具有重要意义。

Objective To explore the relevant factors of the development of non-diabetes macrosomia and their influence on the mother-infant pregnancy prognosis. Methods The clinical materials of 122 non-diabetes macrosomia （observation group） delivered from February 2013 to February 2014 were retrospectively analyzed and compared with those of 150 healthy infants （control group） underwent physical examination in corresponding period. Results The observation group was significantly higher in the mother ages, body mass index （BMI）, increase of BMI in pregnancy and gestational weeks as well as mluripara ratio than the control group （P 〈 0.05 or P 〈 0.01 ）. There were significant differences between two groups in fundal height, waistline, biparietal diameter （BPD） and femur length （FL） of puerpera （P〈0.01）. The pregnancy prognosis showed that there were significant differences between two groups in prolonged labor duration, delivery methods, injury of soft birth canal and post-delivery hemorrhage （P〈 0.05 or P〈 0.01 ）. Conclusion The development of non-diabetes macrosomia is closely associated with the mother ages, BMI, increase of BMI in pregnancy, gestational weeks and parity, and positive implementation of interventional measures is of significance in greatly reducing the rate of non-diabetes macrosomia and improving the health of mothers and infants.
