不同剂量川芎嗪注射液对缺血缺氧性脑病新生儿脑组织损伤的改善作用 被引量:7

摘要 目的探讨不同剂量川芎嗪注射液对缺血缺氧性脑病(HIE)新生儿脑组织损伤的改善作用。方法将87例中、重度HIE新生儿随机分为对照组、大剂量组及小剂量组,各29例。3组均给予相同的常规治疗,大剂量组、小剂量组在此基础上分别加用川芎嗪注射液8 mg/kg、4 mg/kg静脉滴注。结果大、小剂量组各神经症状恢复时间及住院时间均显著短于对照组(P<0.05或P<0.01),且大剂量组各神经症状恢复时间显著短于小剂量组(P<0.05或P<0.01)。3组治疗后超氧化物歧化酶(SOD)均较治疗前升高,一氧化氮(NO)、丙二醛(MDA)、神经元特异性烯醇化酶(NSE)和髓鞘碱性蛋白(MBP)水平较治疗前降低(P<0.05或P<0.01)。组间比较,NO、MDA、NSE和MBP水平,大剂量组<小剂量组<对照组,SOD水平,大剂量组>小剂量组>对照组,差异均有统计学意义(P<0.05或P<0.01)。结论在常规治疗的基础上加用川芎嗪注射液,可有效减轻HIE患儿的脑组织损伤,缓解临床症状,安全性较高,且大剂量川芎嗪的疗效优于小剂量。 Objective To explore the role of different dosages of ligustrazine in improving cerebral tissue impairment of neonates with hypoxic- ischemic encephalopathy (HIE). Methods Eighty- seven neonates with moderate - to severe HIE were randomly divided into control group, large- dosage group and small - dosage group, 29 cases in each group. Three groups were all given the conventional therapy, and large - dosage and small - dosage groups were administrated with ligustrazine injection, 8 mg/kg and 4 mg/kg respectively. Results The recovery time of neurologic symptoms and hospital stays in large - dosage and small - dosage groups were shorter than those of control group, and large - dosage group was shorter than small - dosage group ( P 〈 0.0 5 or P 〈 0.01). The levels of nitric oxide (NO), malondialdehyde (MDA), neuronspecific enolase (NSE) and myelin basic protein (MBP) of three groups decreased more significantly than treatment before (P〈0.05 or P〈0. 01), and the levels of NO, MDA, NSE and MBP in high-dosage group were lower than small - dosage group, followed by control group, while the level of SOD was higher than smalldosage group, followed by control group, the differences were significant among groups ( P 〈 0.05 or P 〈 0.01). Conchtsion The addition of ligustrazine on the conventional treatment can effectively reduce cerebral tissue impairment of HIE neonates, alleviate clinical symptoms with the high safety, and the efficacy of largedosage ligustrazine is better than the small dosage.
