心痛方治疗不稳定型心绞痛的临床疗效观察及其对黏附分子的影响 被引量:5

摘要 目的观察心痛方治疗不稳定型心绞痛(UA)痰瘀互结气滞证的临床疗效,并从抑制黏附分子(ICAM-1、VCAM-1)的表达来探讨心痛方的疗效机制。方法将80例UA患者按随机原则分为治疗组(心痛方加西医常规治疗)和对照组(西医常规治疗),每组40例,疗程均为4周,进行临床疗效观察,包括治疗前后中医证候改变、心绞痛发作的程度、硝酸甘油用量、心电图变化等;并检测UA患者治疗前后细胞间黏附分子-1(ICAM-1)、血管细胞黏附分子-1(VCAM-1)水平。结果治疗组UA患者的临床证候、心电图总有效率均明显优于对照组(P<0.01),治疗组对缓解心绞痛、硝酸甘油停减率、改善中医证候的效果以及降低ICAM-1、VCAM-1水平明显优于对照组(P<0.01)。结论心痛方治疗不稳定型心绞痛痰瘀互结气滞证有良好的临床疗效,能降低ICAM-1及VCAM-1水平,提示该方能干预黏附分子,从而抑制炎症反应。 Objective: To observe the clinical curative effect of Xintong Decoction in treating unstable angina from the inhibition of adhesion molecule expression,and detect into its intervention mechanism on UA. Methods:Patients were randomly divided into the treatment group with Xintong Decoction plus routine western medicine and the control group with routine western medicine. The courses were 4 weeks. Some indicators were observed before and after treatment including clinical curative effect of syndromes,angina pectoris,nitroglycerin consumption changes,ICAM-1 and VCAM-1. Results: The clinical syndrome and the total effective rate in the treatment were superior to those in the control group(P〈0.01). In the treatment group,TCM syndrome score,angina pectoris,nitroglycerin discontinue rate,ECG,ICAM-1,VCAM-1 level were significantly better than those in the control group. Conclusion: Xintong Decoction has good curative effect in treating UA.It can reduce level of ICAM-1and VCAM-1 in patients with UA which could intervene the adhesion molecules to inhibit the inflammatory response.
