口腔种植术后随访2年回顾研究

Retrospective analysis of 2-year follow-up after oral implanted operation

目的:回顾骨结合种植体应用2年的临床效果观察。方法:2011年1月至2012年12月在佛山市中医院口腔种植中心植入的骨结合种植体共298枚,其中ITI系统有266枚,3I系统有32枚,全部种植体均在2013年6月前完成上部结构修复,平均追踪8个月。观察方法为X线检查和临床检查。采用Wheeler存留标准评估,纳入Kaplan2Meier存留曲线统计。结果:种植修复体完成时147例患者对修复效果满意,10例患者出现食物嵌塞,5例修复体松动,3例咬物不适,1例咀嚼无力,1例牙龈美学不满意,1例患者出现术后上颌麻木,4例患者对修复体欠满意,重做后3例表示满意,1例表示可以接受。满意率为87.5%。随访中有8例患者共13枚种植体确诊为种植体周围炎。有明确记录的共4枚种植体脱落。平均8个月的随访中累计修复体松动7件,修复体折断1件,烤瓷冠崩瓷7件,据Wheeler存留标准统计8个月存留率为95.20%。结论:骨结合种植体种植修复后患者的满意度较高,存留率高。

Objective：To observe the clinical effect of osseointegration of implants for 2 years. Method：From 2011 January to 2012 December,there were 298 osseointegrated implants used in the Hospital of Tradi-tional Chinese Medicine（FSTCM）, including 266 cases of ITI system, 32 cases of 3I system. All the im-plants were completed the repair of upper structure before 2013 June and the mean follow-up was 8 month. Result： Of all the patients, 147 patients were satisfied with the treatment effect, but there were food impaction in 10 cases, prosthesis loosening in 5 cases, occlusion discomfort in 3 cases, and 1 case was unable to chew, 1 case was not satisfied with gingival aesthetics, 1 case was maxillary numb after operation, 4 cases were not satisfied with prosthesis （3 cases were satisfied with restoration, 1 cases could accept.）. The tatal satisfactory rate was 87.5%. There were peri implantitis in 13 implants （8 pa-tients）, and 4 implants dropped off. During the mean follow-up time of 8 months, there were prosthesis loosening in 7 cases, prosthesis fracture in 1 case, 7 cases of porcelain. According to Wheeler standard statistics, The survival rate was 95.20% in 8 months （mean time）. Conclusion： The survival rate in os-seointegrated implant restoration is high, which is worth applied.
