经伤椎置钉螺帽松脱19例报告及原因分析

摘要 目的分析19例胸腰段骨折经伤椎置钉内固定后螺帽松脱的可能原因并探讨预防方法。方法我院2009-05-2013-05后路复位经伤椎置钉治疗胸腰椎骨折306例,随访中发现螺帽松脱后记录螺帽松脱部位、发生时间、处理方法及结果等情况,分析失效原因并探讨预防措施。结果 306例经伤椎置钉短节段固定发生螺帽松脱19例,其中11例发现在术后6月之内;本组均为伤椎下位椎体螺帽发生松脱。结论经伤椎置钉治疗胸腰椎骨折出现下位椎体螺帽松脱的原因推测可能与解剖因素、跷跷板效应、钉棒之间耦合欠佳、内固定器械强行复、万向钉角度过大形成应力等相关。
