Expression of EGFR,CEA and p53 in gastric cancer and its clinical significance

摘要 目的:探讨EGFR、CEA和p53在胃癌组织中的表达与临床病理特征之间的关系,并分析其临床意义。方法:应用免疫组织化学方法检测90例胃癌组织中EGFR、CEA和p53的表达,分析EGFR、CEA和p53的表达与临床病理特征的关系。并对64例中晚期胃癌患者进行生存分析。结果:90例胃癌组织标本中EGFR、CEA和p53阳性表达率分别为45.6%、74.4%和53.3%。EGFR的表达与胃癌的分化程度、浸润深度、淋巴结转移和病理分期密切相关(P<0.05);CEA阳性表达与胃癌的浸润深度、淋巴结转移密切相关(P<0.05);p53蛋白表达与胃癌的浸润深度、淋巴结转移和病理分期密切相关(P<0.05)。EGFR、CEA的高表达与患者术后生存期<3年相关(均P<0.05)。EGFR的表达与CEA和p53均呈正相关(r=0.280,P=0.008;r=0.230,P=0.030)。单因素分析显示EGFR、CEA和p53均为影响中晚期胃癌患者预后的重要因素,多因素分析表明EGFR和CEA是影响中晚期胃癌患者预后的独立危险因素。结论:联合检测EGFR、CEA和p53在胃癌组织中的表达有助于胃癌的临床诊断、治疗及预后评估。 Objective:To investigate the clinical value of the exPression of EGFR,CEA and P53 in clinical diagno-sis,treatment and Prognosis in gastric cancer Patients. Methods:The exPression of EGFR,CEA and P53 was detected by immunohistochemical method in samPles of 90 gastric cancers,and analyze the relationshiP of EGFR,CEA and P53 exPression in gastric cancer and clinical features of gastric cancer. 64 cases of Patients with middle-late gastric canc-er were included for survival analysis. Results:The Positive exPression rates of EGFR,CEA and P53 in gastric cancer were 45. 6%,74. 4%,53. 3%. EGFR exPression was significantly correlated with differentiation degree,infiltrative dePth,lymPhatic metastasis and clinicoPathological stage(P〈0. 05). The Positive exPression of CEA was related to the dePth of invasion and lymPhatic metastasis(P〈0. 05). P53 exPression was significantly correlated with infiltrative dePth,lymPhatic metastasis and clinicoPathological stage(P〈0. 05). The EGFR and CEA exPression in gastric canc-er were associated with the survival stage less than three years(all P〈0. 05). EGFR exPression was Positively corre-lated with CEA and P53(r=0. 280,P=0. 008 and r=0. 230,P=0. 030). Univariate analysis showed that EGFR, CEA and P53 were the most imPortant indicators in Prognosis of middle-late gastric cancer. But multivariate analysis revealed that EGFR and CEA were indePendent Prognostic factors for gastric cancer with middle-late stage. Conclu-sion:Combined detection of the exPression of EGFR,CEA and P53 in gastric cancer contributes to the clinical diagno-sis,treatment and Prognosis.
