渐进抗阻肌力训练对与工作相关的慢性颈痛疗效的研究 被引量:16

摘要 目的:观察渐进抗阻肌力训练对与工作有关的慢性颈痛的临床疗效,为指导颈痛患者进行运动训练提供理论依据。方法:将符合入选条件的58例慢性颈痛患者随机分为三组:渐进抗阻训练组(A组)、固定阻力抗阻训练组(B组)和对照组(C组)。三组患者均给予人体工效学指导,训练组给予弹力带颈部肌群前屈、后伸和左/右侧屈抗阻肌力训练,每个动作训练8—12次,每天训练1次,每周至少3次,持续6周。A组训练阻力渐进增加,分别为30%、50%、70%颈部最大等长收缩肌力(MIS),每两周进行1次阻力调整;B组训练阻力固定为入组时70%MIS;C组除外人体工效学指导,不加以其他治疗手段。在患者训练前,训练后2周、4周、6周和3月,分别采用疼痛视觉模拟评分(VAS)、颈椎功能障碍指数(NDI)和颈部MIS进行评估。结果:共49例患者完成训练和随访,其中,A组19例、B组13例和C组17例。三组患者治疗前VAS、NDI和MIS差异无显著性意义(P>0.05)。经过6周训练后,训练组患者VAS、NDI明显改善,MIS增加,分别与C组比较,差异有显著性意义(P<0.05);且A组患者疼痛减轻出现较快,效果明显,与B组比较差异有显著性意义(P<0.05)。结论:抗阻肌力训练可有效缓解与工作有关的慢性颈痛患者的疼痛,改善颈椎功能,增加颈部肌群肌力;采用低强度开始、渐进式抗阻肌力训练可较快减轻疼痛,效果明显,易于接受。
