导乐仪分娩镇痛联合导乐陪伴分娩对产妇分娩过程及结局的影响

Study on the Effect of Daole Labor Analgesia Equipment Combined with Accompany Delivery on the Process and Outcomes of Delivery

目的探讨导乐仪分娩镇痛联合全程责任制陪伴分娩对产妇心理、分娩方式及产后产妇泌乳素的影响。方法将2012年4月至2013年4月于潍坊市妇幼保健院分娩的280例初产妇依据随机数字表法分为观察组和对照组,每组140例。对照组产妇在常规待产室待产,无任何陪伴分娩及分娩镇痛的应用,观察组采用导乐仪分娩镇痛联合全程责任制一对一陪伴分娩。比较两组产妇产时心理反应、最终分娩方式及产后产妇泌乳素的情况。结果观察组不正常心理反应及剖宫产率显著低于对照组,差异有统计学意义(P<0.01);观察组产妇血清泌乳素水平在宫口开全分娩前、产后2 h、产后24 h中均高于对照组,差异有统计学意义(P<0.05)。结论导乐仪分娩镇痛联合责任制陪伴分娩能降低产妇不正常心理反应,降低产妇剖宫产率,增加产妇血清泌乳素水平,从而有利于产后早泌乳和多泌乳,提高母乳喂养率。

Objective To study the effect of Daole labor analgesia equipment combined with full responsibility of delivery accompany on maternal psychology,way of delivery and postpartum maternal prolactin. Methods A total of 280 primiparas delivered in Weifang Maternal and Children＇ s Health Care Center from Apr. 2012 to Apr. 2013 were divided into observation group and control group according to random number table,with 140 cases in each group. Patients in the control group stayed in the conventional labor room without doula or labor analgesia,while patients in the observation group were adopted Daole labor analgesia equipment combined with one-to-one doula accompany delivery. Abnormal psychological reactions,ways of delivery and condition of postpartum prolactin of the two groups were compared. Results The incidence of abnormal psychological reactions and cesarean section rate of the observation group were significantly lower than those of the control group,the differences were statistically signiicant（ P〈0. 01）; serum prolactin levels before orifice of uterus fully opened,and 2 h,24 h after delivery of the observation group were all higher than those of the control group,there were statistically significant differences between two groups（ P〈0. 05）. Conclusion Painless Daole labor analgesia equipment combined with doula accompany delivery can reduce abnormal maternal psychological reactions and cesarean section rate,and increase the postpartum prolactin levels of the puerperal,so as to contribute to the early and more lactation and increase the rate of breastfeeding.
