原发性肾病综合征与血小板活化因子水解酶的相关性分析 被引量:5

摘要 目的探讨原发性肾病综合征(PNS)与血小板活化因子水解酶(PAF-AH)的相关性。方法选择湖北省黄石市中心医院2013年1—6月收治的PNS患者48例,按病理类型分为单纯性肾病综合征(STNS)组31例及肾炎型肾病综合征(NTNS)组17例,并给予6个月激素治疗后根据对激素治疗效应差异分为激素敏感型肾病综合征(SSNS)组19例,激素耐药型肾病综合征(SRNS)组15例,激素依赖型肾病综合征(SDNS)组14例;选择同期健康体检人员30例作为对照组。比较各组间PAF-AH活性,分析其与不同类型PNS的相关性。结果 PAF-AH活性STNS组均高于NTNS组及对照组,且NTNS组高于对照组(P<0.05);SSNS组均高于SRNS组、SDNS组、对照组,且SRNS组、SDNS组均高于对照组(P<0.05)。PAF-AH活性与STNS、NTNS、SSNS、SRNS、SDNS呈正相关(r=0.618、0.524、0.717、0.567、0.327,P<0.05)。结论 PNS患者PAF-AH活性显著高于正常人群,且不同类型PNS患者PAF-AH活性不同,可能是该病对激素治疗反应不一致的原因。 Objective To investigate the correlation of primary nephrotic syndrome(PNS) and platelet activating factor acetylhydrolase( PAF-AH). Methods A total of 48 PNS patients from January to June 2013 were divided into simple type nephrotic syndrome group( STNS group,n = 31) and nephritic type nephrotic syndrome group( NTNS group,n = 17) according to pathological types. After hormonal therapy for 6 months,the 48 patients were divided into steroid-sensitive nephritic syndrome group( SSNS group,n = 19),steroid-resistant nephritic syndrome group( SRNS group,n = 15) and steroid-dependent nephritic syndrome group( SDNS group,n = 14) according to the effect differences in hormonal therapy,and then 30 healthy persons taking medical examination at the same period were selected as a control group. The PAF-AH activity in each group was determined,and then its correlations with different types of PNS were analyzed. Results The value of PAF-AH activity in STNS group was significantly higher than that in NTNS group,and the activity value in NTNS group was significantly higher than that in control group( P〈0. 05); the activity value in SSNS group was significantly higher than those in SRNS,SDNS and control groups,and the activity values in SRNS and SDNS groups were significantly higher than that in control group( P〈0. 05). The activity of PAF-AH was positively correlated in STNS,NTNS,SSNS,SRNS and SDNS groups( r = 0. 618,0. 524,0. 717,0. 567,0. 327,P〈0. 05). Conclusion The values of PAF-AH activity in PNS patients are significantly higher than those in normal people,and the different types of PNS patients have different PAF-AH activities,which may explain why there are different responses to hormonal therapy.
