内囊后肢梗死的分水岭机制探讨 被引量:6

摘要 目的:探讨内囊后肢梗死可能的分水岭机制。方法回顾性分析2009年1月至2013年9月,入住我院全部10例内囊后肢梗死患者的临床表现和影像学资料。结果7例表现为偏侧面部及上下肢无力,3例表现为上下肢无力;3例表现为24h内多次短暂性脑缺血发作,2例表现为5h以内达高峰,5例表现为3~5d内病情进展。磁共振扩散加权成像(diffusion weighted imaging,DWI)病灶部位:均在内囊后肢可见DWI高信号,其中1例可见双侧内囊后肢高信号,仅1侧肢体出现临床症状。6例在病灶侧侧脑室体后部旁,同内囊后肢相隔0.7 cm之内可见另一病灶。头部磁共振血管成像(magnetic resonance angiography,MRA)显示均有后循环供血差表现,其中1例表现为基底动脉迂曲,1例表现为基底动脉开窗畸形伴有病灶侧大脑中动脉M1段狭窄。结论内囊后肢是脑血液前后循环的分水岭,椎基底动脉狭窄是内囊后肢梗死的重要原因。
