有创机械通气辅助治疗中老年患者重症急性左心衰竭疗效分析

Invasive Mechanical Ventilation Treatment of Severe Acute Left Heart Failure in Elderly Patients with Curative Effect Analysis

目的观察有创机械通气辅助治疗中老年患者重度急性左心衰竭的临床效果。方法回顾分析我院重症医学科37例重度急性左心衰竭患者在常规治疗无效的情况下,行经口气管插管和机械通气治疗,比较治疗前后患者的临床症状、心率、血压、呼吸频率、血气分析、脑钠肽、左心室射血分数等主要指标的变化。结果机械通气3h患者动脉血气氧分压和二氧化碳分压、心率和呼吸频率、血氧饱和度、血压等各项检测指标较机械通气前明显改善。37例患者中34例成功撤机,总有效率91.89%.结论气管插管后机械通气治疗重度急性左心衰竭能迅速缓解患者临床症状,可有效提高抢救成功率。

Objective To observe the adjuvant treatment of invasive mechanical ventilation in elderly pa-tients with severe acute left heart failure.Methods Retrospective analysis of intensive medicine and clinical results of 37 cases of severe acute left heart failure patients with invalid conventional treatment,through the breath tube intubation and mechanical ventilation treatment,the clinical symptoms,heart rate,blood pressure,breathing rate,blood gas analysis, brain natriuretic peptide and the change of the main indexes such as left ventricular ejection fractionin patients before and after treatment were compared.Results After mechanical ventilation for 3 hour arterial blood gas oxygen partial pressure and CO2 partial pressure,the detection indicators of patients such as heart rate and breathing rate,blood oxygen saturation,blood pressure and so on improved significantly before the mechanical ventilation.Among 37 cases,34 cases were successfully withdrawed machine,the total effective rate was 91.89%.Conclusion Endotracheal intubation after mechanical ventilation treatment of severe acute left heart failure can quickly relieve the clinical symptoms ,which can ef-fectively improve the success rate of rescue.
