前臂远端双干型动脉化静脉皮瓣在手指皮肤软组织缺损中的应用

Using Double Trunks Arterialized Venous Free Flap to Repair Finger Soft Tissue

目的探讨前臂远端双干型动脉化静脉皮瓣修复手指皮肤软组织缺损的疗效。方法 2014年3月~2014年9月收治手指软组织缺损患者5例,致伤原因:机器挤压伤4例,重物砸伤1例,均为手指皮肤软组织缺损。损伤部位:食指3例,环指2例,受伤至入院时间30min^2h,平均1.5h。皮肤软组织缺损面积为1.5cm×2.0cm^2.0cm×3cm,应用前臂远端双干型动脉化静脉皮瓣进行修复。结果术后5例皮瓣均获得存活,其中1例早期出现颜色发暗、散在小水疱形成,后期恢复可。结论应用前臂远端双干型动脉化静脉皮瓣修复手指皮肤软组织缺损,皮瓣来源广泛,切取方便,对主要血管损伤小,术后外观良好。

Objective To observe the effectiveness of using double trunks arterialized venous free flap to repair finger soft tis-sue.Methods From March 2014 to September 2014,3 cases of finger soft tissue were treated.Machine crush injury were 4 cases and bruise was 1 case.The locations were index finger in 3 cases,ring finger in 2 cases.The duration of injury and adminssion was 0.5~2 hours.The size of defects ranged from 1.5cm ×2cm to 2.0cm ×3cm.All defects were repaired by forearm arterialized venous flap.Results Five flaps all survived,dulling and blister were observed in 1 flap and it got well later.Conclusion It is feasible to repair finger tissue to using arterial-ized venous free flap.It has the advantages of flap sources,small major vascular injury and good postoperative appearance.
