摘要
目的 观察微小RNA(miRNA,miR)-219-5p与乙醇、临床病理特征的关系及其在肝细胞肝癌(HCC)发生发展中的作用.方法 采用实时荧光定量聚合酶链反应(FQ-PCR)法对120例HCC患者组织中的miR-219-5p的表达进行检测,并比较其与饮酒、临床病理特征之间关系.结果 (1)肿瘤瘤体>5 cm、组织分化差以及大量饮酒者miR-219-5p的表达显著降低(x2=10.550、3.956、6.061,P<0.05);(2)在Logistic回归分析中,大量饮酒史是miR-219-5p表达水平下降的主要危险因素之一[95%可信区间(CI):1.521 ~ 17.635,P<0.05];(3)在Cox回归模型中,miR-219-5p表达水平降低的患者具有较高的死亡风险,饮酒、甲胎蛋白(AFP)阳性、慢性肝病以及肿瘤较大者,死亡风险进一步增加(P<0.05).结论 miR-219-5p表达水平、血清AFP阳性、肿瘤大小、慢性肝病是HCC患者生存预后的独立危险因素,同时大量饮酒可以进一步放大上述效应.
Objective To study the roles of microRNA (miRNA,miR)-219-5p in the pathogenesis of hepatocellular carcinoma (HCC) and its relationship with alcohol and clinicopathological features.Methods The expression of miR-219-5p in 120 cases of HCC tissue samples was detected by real-time fluorescent quantitative polymerase chain reaction (FQ-PCR),and its relationship with alcohol and clinicopathological features was analyzed.Results (1) The expression of miR-219-5p was decreased significantly in those cancers tumors with diameter > 5 cm,worse tissue differentiation and heavy drinking (x2 =10.550,3.956,and 6.061 respectively,P < 0.01);(2) The logistic regression analysis revealed thaat a history of heavy drinking was one of the major risk factors of down-regulated miR-219-5p expression level [95% confidence interval (CI):1.521-17.635,P <0.05);(3) In the Cox regression model,the patients with the down-regulated miR-219-5p expression levels had high risk of death,and drinking,positive alpha fetal protein (AFP),chronic liver disease and larger tumor further increased the risk for death (P < 0.05).Conclusion It is believed that the expression of miR-219-5p,serum AFP-positive,tumor size,and chronic liver disease are independent risk factors for survival of patients with HCC,while heavy drinking consumption can further amplify the effects.
