结肠癌临床病理特征与Toll样受体4表达的关系 被引量:4

Relationship and significance of Toll-like receptor 4 expression with clinic-pathological characteristics of colon cancer

摘要 目的 检测结肠癌、溃疡性结肠炎及癌旁肠黏膜组织中Toll样受体4(TLR-4)的表达,探讨TLR-4与结肠癌发生、发展及预后的关系.方法 采用实时荧光定量聚合酶链反应(FQ-PCR)法对247例结肠癌患者癌组织、48例相对正常组织、50例溃疡性结肠炎组织中TLR-4mRNA进行检测.结果 癌组织TLR-4表达水平与淋巴结转移状态、有无远处转移、临床分期、组织学类型等临床病理特征明显相关(P<0.05).高表达TLR-4 mRNA患者与低表达者比较,中位总生存期(OS)明显缩短(24个月比52个月,P<0.01).结肠癌组织中TLR-4 mRNA的表达水平明显高于溃疡性结肠炎组织和癌旁组织(P<0.01).溃疡性结肠炎组织中TLR-4 mRNA表达水平明显高于癌旁组织(P<0.01).结论 结肠癌组织中TLR-4表达与临床分期和肿瘤转移相关.TLR-4高表达预示着结肠癌患者预后不良. Objective To investigate the expression level Toll-like receptor 4 (TLR-4) mRNA in colon cancer tissues,ulcerative colitis and normal tissues,and to explore the significance of TLR-4 mRNA in tumorigenesis,progression and prognosis of colon cancer.Methods The mRNA of TLR-4 was detected by real-time fluorescent quantitative polymerase chain reaction (FQ-PCR) in 247 cases of cancer tissues,50 cases of ulcerative colitis tissues and 48 cases of normal tissues.The level of TLR-4 mRNA relative expression between groups were analyzed by Mann-Whitney-Wilcoxon U-Test.Survival curves were plotted using Kaplan-Meier method with Log-rank test.Results The expression level of TLR-4 mRNA in colon cancer tissues were closely related to lymph node status,presence of distant metastasis and clinical stages (P < 0.05),but not to the Karnofsky Performance Score (KPS) and histological type of the patient (P > 0.05).The median OS between patients with high levels of TLR-4 expression and low levels of TLR-4 expression had statistically significant difference (24 months vs.52 months,P < 0.01).Conclusion High expression of TLR-4 correlated with clinical stages and tumor metastasis.High expression of TLR-4 indicated a poor prognosis of colon cancer patients.
