摘要
目的探讨灰阶超声和弹性成像及二者联合应用在良、恶性甲状腺肿块鉴别诊断中的应用效果。方法回顾性分析2010年1月—2013年12月接诊的38例甲状腺肿块患者。采用灰阶超声以及弹性成像对患者进行检查。以术后病理结果作为金标准。比较不同方法检查的准确率、ROC曲线下的面积、面积的标准误以及95%的可信区间。结果灰阶超声的准确率(72.00%)明显低于弹性成像的准确率(84.00%),两者比较有统计学意义(P<0.05);二者联合检查的准确率(94.00%)明显高于灰阶超声和弹性成像的准确率(P<0.05);ROC曲线中,弹性成像曲线下的面积(0.815)明显优于灰阶超声(0.733),两者比较有统计学意义(P<0.05),二者联合应用曲线下的面积优于灰阶超声和弹性成像独自应用(P<0.05)。结论灰阶超声和弹性成像二者联合应用在良、恶性甲状腺肿块鉴别诊断中的应用效果良好,值得临床推广。
Objective To investigate application results of gray - scale ultrasound and elastography and combined application of both for diagnosis of benign and malignant thyroid nodules. Methods Totally 38 cases with thyroid nodules from January 2010 to December 2013 in our hospital were researched. Using gray scale ultrasound, elastography and combined application of both to diagnosis and compared their results with pathology results. Comparison of different methods' accuracy, the area under the ROC curve, standard error of the area and 95% confidence intervals. Results The accuracy of grayscale ultrasound was 72.00%, it was significantly lower than elastography accurately rate (84. 00% ),there were statistically significant (P 〈 0. 05 ); combined application of grayscale ultrasound with elastograph was accuracy (94. 00% ) , it was significantly higher than accuracy rate gray - scale ultrasound and elastography ( P 〈 0. 05 ) ; the area under the curve of elastography is 0. 815, it was significantly better than the gray scale ultrasound (0. 733 ), there were statistically significant (P 〈 0. 05 ), Area under the curve of combined application of grayscale ultrasound with elastograph was superior than gray scale ultrasound and elastography, there were statistically significant (P 〈 0.05 ). Conclusions Application results of combined application of combined application of grayscale ultrasound with elastograph for diagnosis of benign and malignant thyroid nodules is well. It is worthy of promotion in clinic.
