彩色多普勒在大脑中动脉血流动力学异常胎儿预后评估中的应用

Color Doppler to the Middle Cerebral Artery blood flow mechanics abnormal fetal prognosis prediction value

【目的】探讨彩色多普勒血流显像（ CDFI ）在大脑中动脉血流动力学异常胎儿预后评估中的意义。【方法】选择产前 CDFI 诊断胎儿大脑中动脉血流动力学异常的孕妇190例，随访6个月至分娩，根据其妊娠结局，将其分为预后不良组（82例）及预后良好组（108例），比较两组胎儿大脑中动脉血流参数：收缩期峰值血流速度／舒张末期血流速度（S／D）、阻力指数（resistance index ，RI）。【结果】预后不良组 S／D 、RI 值均显著低于预后良好组，差异有统计学意义（ P ＜0．05）；轻度预后不良组与预后良好组大脑中动脉血流参数值差别无统计学意义（ P ＞0．05），中、重度预后不良组均显著低于轻度预后不良组（ P ＜0．05），但中、重度预后不良组比较无统计学意义（ P ＞0．05）。【结论】采用CDFI 检测胎儿大脑中动脉血流参数，对监测胎儿宫内状况、预测胎儿预后、决定胎儿最佳娩出时机具有一定的临床价值。

[Objective]To study Color Doppler to the middle cerebral artery hemodynamic abnormal fetal prognosis pre ‐diction .[Methods]There were 190 cases with the fetal Umbilical Arterial blood flow anomalies in our hospital ,and the average gestational age was （37 .77 ± 5 .76 ）weeks .To observe pregnancy outcomes ,the patients were followed up ranging from 6 months to four years .Finally the fetuses were split into two parts ：poor prognosis group and good prognosis group .Combined with the pregnancy prognosis ,there conducted a retrospective analysis of the relationship between fetal middle cerebral artery blood flow characteristics and prognostic parameters .[Results]There were 108 cases in good group and 82 cases in poor group , Compared to the good group ,S/D ,RI of the poor group were significantly lower than the good prognosis group ,the difference was statistically significant （P 〈 0 .05） ;The blood flow parameters of the good group and mid poor group had no difference significant（ P 〉 0 .05） ,The blood flow parameters of the good group and moderate ,severe poor group had difference signifi‐cant（ P 〈 0 .05 .） .[Conclusion]There is clinical value for using color doppler to measure the middle cerebral artery blood flow parameters of fetal as to monitor fetal status ,predict fetal pregnancy prognosis and the best delivery time .
