大肠俞募配穴针刺治疗功能性便秘的随机对照研究

A clinical randomized controlled trial of acupuncture at the combination of back-shu point and front-mu point of large intestine meridian in the treatment of functional constipation

目的观察针刺大肠俞募配穴(天枢配大肠俞)及非经非穴点应用于功能性便秘(functional constipation,FC)的疗效,探讨俞募配穴法治疗FC的机制,为针刺治疗本病选穴方案提供临床依据。方法将72例FC患者,按随机数字法分为治疗组和对照组各36例。治疗组针刺天枢穴及大肠俞穴,对照组针刺非经非穴。两组均每日针刺1次,每次30 min,每周治疗5次为一个疗程,连续治疗4个疗程。分别于治疗前、治疗完成时比较两组疗效。结果治疗组在增加排便频率及改善排便困难方面优于对照组,差异有统计学意义(P<0.05);两组总有效率、粪便性质、排便时间、下坠不尽感、腹胀积分和总积分比较,差异无统计学意义(P>0.05)。结论大肠俞募配穴与非经非穴点针刺均能有效治疗FC,改善患者的症状,但前者便秘症状的改善程度优于后者。

Objective To observe the efficiency of acupuncture at acupoints of the ST25 matching with the B25 in the treatment of patient with functional constipation（FC） and to compare with that of unspecific acupuncture in order to provide clinical basis for optimizing the treatment.Methods Seventy-two patients were randomly divided into a treatment group and a control group,36 in each group.The treatment group was treated with acupuncture at acupoints of the ST25 matching with B25（the combination of back-shu point and front-mu point of large intestine meridian） while the control group was treated by an unspecific acupuncture.All patients received a total of four treatment cycles,each of which consisted of acupuncture once a day（0.5 h each time） for one week（five times）.Immediately before and after the treatment,the effects were assessed.Results At the end of the treatment,the two groups had statistical differences in stool frequency and difficult defecation（P〈0.05）.However,no statistically differences in total effective rate,stool property,defecation time,tenesmus,abdominal distention and total score（P〉0.05）.Conclusion Acupuncture at the back-shu point matching with the front-mu point of large intestine meridian and unspecific points are both effective in the treatment of FC patients.However,the efficacy of acupuncture at the back-shu point matching with the front-mu point of large intestine meridian is superior to those of unspecific acupuncture in improving the symptoms of FC patients
